Cross-sectional study of patients with Trigeminal Neuralgia treated at the Walter Cantídio Hospital of the Federal University of Ceará

Resumo: Trigeminal Neuralgia is described as excruciating pain, similar to an electric shock that affects one or more branches of the trigeminal nerve. The treatment of Trigeminal Neuralgia (TN) remains a therapeutic challenge. Pharmacological treatment is the first choice, however many patients become intolerant to it, being referred to surgery. Thus, this study aimed to evaluate data from a group of patients diagnosed with trigeminal neuralgia (TN) treated at the Walter Cantídio University Hospital of the Federal University of Ceará, focusing on sociodemographic data, symptoms and treatment type. The following data were collected on age, sex, anamnesis, reported symptoms, diagnosis, comorbidities, treatment and evolution. Data from 62 patients obtained from medical records during the follow-up period were included. The results showed that the incidence in all age groups was primarily in females, the most reported symptom was unilateral shock pain. 25% of patients associated the onset of symptoms with a previous event such as tooth extraction, endodontic treatment or trauma. The drug of choice for treatment was carbamazepine in 74% of patients, 20% of patients did not control pain with treatment and underwent surgery. Therefore, more advances in scientific studies regarding pain mechanisms and the discovery of more precise drugs to control it will be the way to treat trigeminal neuropathic pain.

El tratamiento de la Neuralgia del Trigémino (NT) sigue siendo un desafío terapéutico. El tratamiento farmacológico es de elección, sin embargo, muchos pacientes se vuelven intolerantes a la medicación, siendo encaminados para cirugía. Así, este estudio tuvo como objetivo evaluar os datos de un grupo de pacientes con diagnóstico de neuralgia del trigémino (TN) atendidos en el Hospital Universitario Walter Cantídio de la Universidad Federal de Ceará, con foco en datos sociodemográficos, síntomas y tipo de tratamiento. Se recogieron los siguientes datos sobre edad, sexo, anamnesis, síntomas referidos, diagnóstico, comorbilidades, tratamiento y evolución. Se incluyeron datos de 62 pacientes obtenidos con las historias clínicas durante el período de seguimiento. Los resultados mostraron que en todos los grupos la incidencia fue principalmente en el sexo femenino, el síntoma más informado fue el dolor de choque unilateral. El 25% de los pacientes asoció el inicio de los síntomas con un evento previo como extracción dental, tratamiento de endodoncia o traumatismo. El medicamento de elección para el tratamiento fue la carbamazepina en el 74% de los pacientes, el 20% de los pacientes no controlaron el dolor con el tratamiento y fueron intervenidos quirúrgicamente. Por lo tanto, más avances en los estudios científicos sobre los mecanismos del dolor y el descubrimiento de fármacos más precisos para controlarlo serán la forma de tratar el dolor neuropático del trigémino.A Neuralgia do Trigêmeo é descrita como uma dor excruciante, semelhante a um choque elétrico que acomete um ou mais ramos no nervo trigêmeo. O tratamento da Neuralgia do Trigêmeo (NT) continua sendo um desafio terapêutico. O tratamento farmacológico é de escolha, no entanto muitos pacientes tornam-se intolerantes a medicação, sendo encaminhados a cirurgia. Assim, este estudo teve por objetivo avaliar os dados de um grupo de pacientes diagnosticados com neuralgia do trigêmino (NT) atendidos no hospital universitário Walter Cantídio da Universidade Federal do Ceará, focando dados sociodemográficos, sintomatologia e tipo de tratamento. Foram coletados os seguintes dados de idade, sexo, anamnese, sintomas relatados, diagnóstico, comorbidades, tratamento e evolução. Foram incluídos os dados de 62 pacientes obtidos dos prontuários durante o período de acompanhamento. Os resultados mostraram que a incidência em todos as faixas etárias foi prioritariamente no gênero feminino, a sintomatologia mais relatada foi a dor em choque, unilateral. 25 % dos pacientes associaram o início dos sintomas a um evento antecessor como exodontia, tratamento endodôntico ou trauma. A medicação de escolha para o tratamento foi a Carbamazepina em 74 % dos pacientes, 20% dos pacientes não controlaram a dor com o tratamento e foram submetidos a cirurgia.
