Pre-exposure prophylaxis to HIV/AIDS: situational analysis after 03 years of availability in the Unified Health System (SUS)

Resumo: To assess adherence to pre-exposure prophylaxis (PrEP) to HIV after 3 years of implementation in the Unified Health System (SUS). Ecological, quantitative study, carried out in 2021, in Brazil. Data on HIV PrEP were collected in the monitoring panel provided by the Ministry of Health – Federal Government. The variables studied were type of population, age group, education, race/color; number of dispensations in Brazil from January 2018 to February 2021, number of health services offering PrEP, discontinuation of PrEP, number of new users per month, number of active users per month and sexual habits. Data were tabulated in excel and descriptive statistical analysis was performed for all variables, the chi-square test to verify the association between the variables "discontinuity of PrEP" and "type of key population" and the binomial test of two proportions to compare sexual habits in the first and last consultations. Most of the population is gay or men who have sex with men (82.6%), age group between 30 and 39 years old (51%), white or yellow color (57.2%), 12 years or more of schooling (71%). 42% of subjects discontinued treatment at some point. There was a statistically significant association between the variables “discontinuity of PrEP” and “type of key population”, and a decrease in condom use and in the number of sexual partners between the first and last consultations. Although adherence to the method is increasing, discontinuity in treatment among users is high, which makes success in effectiveness difficult.

Estudio ecológico, cuantitativo, realizado en 2021, en Brasil. Los datos sobre la PrEP para el VIH se recopilaron en el panel de monitoreo proporcionado por el Ministerio de Salud - Gobierno Federal. Las variables estudiadas fueron tipo de población, grupo de edad, escolaridad, raza/color; número de dispensaciones en Brasil de enero de 2018 a febrero de 2021, número de servicios de salud que ofrecen PrEP, discontinuación de PrEP, número de nuevos usuarios por mes, número de usuarios activos por mes y hábitos sexuales. Los datos fueron tabulados en excel y se realizó análisis estadístico descriptivo para todas las variables, la prueba de chi-cuadrado para verificar la asociación entre las variables “discontinuidad de PrEP” y “tipo de población clave” y la prueba binomial de dos proporciones para comparar hábitos sexuales. en la primera y última consulta. La mayoría de la población es gay u hombres que tienen sexo con hombres (82,6%), grupo de edad entre 30 y 39 años (51%), color blanco o amarillo (57,2%), 12 años o más de escolaridad (71%) . El 42% de los sujetos interrumpieron el tratamiento en algún momento. Hubo una asociación estadísticamente significativa entre las variables “discontinuidad de la PrEP” y “tipo de población clave”, y una disminución en el uso del preservativo y en el número de parejas sexuales entre la primera y la última consulta. Aunque la adherencia al método está aumentando, la discontinuidad en el tratamiento entre los usuarios es alta, lo que dificulta el éxito en la efectividad.Avaliar a  adesão à profilaxia pré-exposição (PrEP) ao HIV após 3 anos de implantação no Sistema Único de Saúde (SUS).  Estudo ecológico, quantitativo, realizado no ano de 2021, no Brasil.  Foram coletados dados a respeito da PrEP HIV, no painel de monitoramento disponibilizado pelo Ministério da Saúde – Governo Federal. As variáveis estudadas foram tipo de população, faixa etária, escolaridade, raça/cor; número de dispensações no Brasil de Janeiro de 2018 a Fevereiro de 2021, número de serviços de saúde que oferecem a PrEP, descontinuidade da PrEP, número de novos usuários ao mês, número de usuários ativos ao mês e hábitos sexuais. Os dados foram tabulados no excel e foi realizada a análise estatística descritiva para todas as variáveis, o teste qui-quadrado para verificar a associação entre as variáveis “descontinuidade da PrEP” e “tipo de população-chave” e o teste binomial de duas proporções para comparar os hábitos sexuais no primeiro e último atendimento. A maior parte da população são gays ou homens que fazem sexo com homens (82,6%), faixa etária entre 30 e 39 anos (51%), raça branca ou amarela (57,2%), 12 anos ou mais de escolaridade (71%). 42% dos indivíduos interromperam o tratamento em algum momento. Houve associação estatisticamente significante entre as variáveis “descontinuidade da PrEP” e “tipo de população-chave”,  e diminuição no uso de preservativo e no número de parcerias sexuais entre a primeira e última consulta.
