Resumo: | Medicines are important drives of health expenditure. To ensure that all patients have access to this technology is challenging given the financial constraints of health systems. The aim of this study was to analyze drug expenditures and their drivers in a public hospital located in Rio de Janeiro, Brazil, from 2013 to 2020. All data regarding drug purchases were retrieved from the database of the hospital Information System used to manage health products purchases. The pharmaceutical products were classified in accordance with the Anatomical, Therapeutic, and Chemical (ATC) classification system coordinated by the WHO Collaborating Centre for Drug Statistics Methodology and aggregated by volume and by expenditure. The expenditure variation was decomposed into three broad categories: price effects, quantity effects, and drug mix effects. During the period, annual expenditure reduced by 20%, ranging from from R$ 16.9 million in 2013 to R$ 13.4 million in 2020, while purchased quantities also dropped by 33.7%, ranging from 5.7 million in 2013 to 3.8 million unites in 2020. Two factors drove the reducition of expenditures, the quantity effect and the price effect. These findings may contribute to better understanding medicine spending trends and the factors that influence them in order to formulate strategies to optimize resource utilization and improve patients access to the necessary tratments. |

El objetivo de este estudio fue analizar los gastos de los medicamentos en un hospital público en Río de janeiro entre 2013 y 2020. Los datos de adquisición se obtuvieron a través del Sistema de Información de Gestión y Control de Inventarios (SIGCE) del hospital. Los medicamentos se clasificaron según el sistema OMS-ATC/DDD y se agregaron por cantidad y por gasto. Se realizó un análisis de descomposición para los medicamentos de la clase A, entre 2015 y 2020. Los gastos anuales se redujeron cerca de un 20% en el período (de R$ 16,9 millones en 2013 a R$ 13,4 millones en 2020), y la cantidad total comprada también se redujo un 33,7%, pasando de 5,7 millones de unidades en 2013 a 3,8 millones en 2020. Lo precios y la cantidad contribuyeron a la reducción de los gastos, mientras que el mix de medicamentos contribuye positivamente, optando por la compra de artículos, en promedio, más caros. Los antineoplásicos, inmunosupresores y antiinfecciosos utilizados por el sistema fueron los mayores gastos en la clase A y juntos representaron el 68,6% del gasto total de la clase, con destaque para el subgrupo L01F (anticuerpos monoclonales), además del subgrupo J02A (antimicóticos de uso sistémico). Este estudio mostró que la tendencia a la baja en el gasto en medicamentos estuvo determinada por la disminución de la cantidad comprada de artículos con el precio unitario más alto.Os medicamentos são um dos principais fatores das despesas em saúde e garantir o acesso a essa tecnologia é um desafio diante da limitação financeira dos sistemas de saúde. O objetivo desse estudo foi analisar os gastos com medicamentos de um hospital federal terciário do Rio de janeiro entre 2013 e 2020. Dados de aquisição foram obtidos através do Sistema de Informação de Gestão e Controle de Estoque (SIGCE) do hospital. Os medicamentos foram classificados de acordo com o sistema WHO-ATC / DDD e agregados por quantidade e por gasto. Realizou-se análise de decomposição para os medicamentos da curva A, entre os anos 2015 e 2020. Os gastos anuais reduziram-se cerca de 20% no período (de R$ 16,9 milhões em 2013 para R$ 13,4 milhões em 2020), e a quantidade total adquirida também sofreu queda de 33,7%, variando de 5,7 milhões de unidades em 2013 para 3,8 milhões em 2020. Os preços e a quantidade contribuíram para a redução dos gastos, enquanto o mix de medicamentos contribui positivamente, optando-se pela aquisição de itens, em média, mais caros. Os agentes antineoplásicos, imunossupressores e anti-infecciosos de uso sistêmico foram os maiores gastos na classe A e juntos representaram em média 68,6% do gasto total da classe, destacando-se o subgrupo L01F (anticorpos monoclonais), além do subgrupo J02A (antimicóticos de uso sistêmico).
