Surgical treatment for odontogenic keratocyst and analysis of its imaging and histopathological characteristics: Case report

Resumo: | The Odontogenic Keratocyst (OK) is a lesion that affects the bones of the mandible and maxilla, of slow growth and asymptomatic. It originates from the odontogenic tissue by the proliferation of mesenchymal tissue, epithelial tissue, or both, with keratin production inside. OK treatment takes place in different stages and therapeutic approaches, therefore, the objective of this article is to describe, through a case report, the treatment performed for an OK from the completion of marsupialization, the surgical approach performed, and the two-year postoperative follow-up in a 15-year-old male patient. The lesion was radiolucent, unilocular, with a radiopaque halo, and anteroposterior growth without destruction of the cortical bone and the initial clinical approach consisted of performing an aspiration puncture and incisional biopsy whose histopathological analysis confirmed the diagnosis of OK. Therefore, marsupialization was performed with a cannula to stimulate the bone neoformation process and reduce the size of the lesion to be favorable to completely remove it, in addition to providing a safety margin to the adjacent noble structures. Six months after the decompression of the lesion, the patient underwent surgical enucleation of the lesion associated with peripheral osteotomy. The radiographic aspect, 2 years after the surgery, shows locais bone neoformation, the integrity of noble structures such as the mandibular canal, and no evidence of lesion recurrence. The patient will be followed up for another 3 years, every 6 months. |

Se origina a partir del tejido odontogénico por proliferación de tejido mesenquimal, tejido epitelial o ambos, con producción de queratina en su interior. El tratamiento de los QO se desarrolla en diferentes etapas y abordajes terapéuticos, por lo que el objetivo de este artículo es describir, a través de un reporte de caso, la tratamiento realizado a un QO desde la finalización de la marsupialización, el abordaje quirúrgico realizado y el seguimiento postoperatorio a los dos años en un período de 15 años. anciano paciente masculino. El paciente será seguido durante otros 3 años, cada 6 meses.O Ceratocisto Odontogênico (CO) é uma lesão que acomete os ossos da mandíbula e maxila, de crescimento lento e assintomático. Se origina do tecido odontogênico por proliferação do tecido mesenquimal, epitelial, ou de ambos, com produção de ceratina em seu interior. O tratamento do CO acontece em diferentes etapas e abordagens terapêuticas, portanto, o objetivo desse artigo é descrever, através de um relato de caso, o tratamento realizado para um CO desde a conclusão da marsupialização, abordagem cirúrgica realizada e acompanhamento pós-operatório de dois anos em um paciente masculino de 15 anos. A lesão apresentava-se radiolúcida, unilocular, com halo radiopaco, crescimento anteroposterior sem destruição da cortical óssea e a abordagem clínica inicial consistiu na realização de punção aspirativa e biopsia incisional cuja análise histopatológica confirmou o diagnóstico de CO. Sendo assim, realizou-se a marsupialização com cânula a fim de estimular o processo de neoformação óssea e reduzir o tamanho da lesão para ser favorável removê-la completamente, além de conferir uma margem segurança às estruturas nobres adjacentes. Com seis meses de descompressão da lesão, o paciente foi submetido a enucleação cirúrgica da lesão associada a osteotomia periférica. O aspecto radiográfico, dois anos após a cirurgia, mostra neoformação óssea local, integridade de estruturas nobres como o canal mandibular e sem evidência de recidiva da lesão.
