Hemodynamic and respiratory changes from a continuous infusion of dexmedetomidine in general anesthesia in dogs: a systematic review and meta-analysis

Resumo: | Dexmedetomidine uses widely in veterinary medicine. This drug achieves analgesic, sedative, and muscular relaxation, and can be administered intravenously in continuous infusion with or without an initial bolus. But cardiovascular and respiratory alterations had been considered. This review aimed to analyze the effect of cardiovascular and respiratory in dogs under continuous infusion of dexmedetomidine. For the selection of studies, words like dogs, dexmedetomidine, and continuous infusion, were inserted into the Pubmed and Embase platforms. And 11 studies were admitted, with pre-established inclusion and exclusion criteria, to perform the meta-analysis. The articles used dexmedetomidine at doses ranging from 0.1-4.5 µg/kg and different associations. The reduction of general anesthetics with dexmedetomidine alone was 11% to 69%. And when associated with another drug can reduce the general anesthetic by 86%. In the cardiovascular system, dexmedetomidine increased mean arterial pressure and decreased heart rate in a dose-dependent (P < 0.00). There was also a decrease in cardiac output and index (P < 0.00). In respect of respiratory parameters, there were no significant differences in respiratory rate, carbon dioxide, and oxygen in arterial blood. In conclusion, dexmedetomidine decreases the consumption of general anesthetics. Increases mean arterial pressure while reducing heart rate, output, and cardiac index in a dose-dependent manner without significantly altering respiratory function. |

El objetivo de esta revisión fue analizar los efectos cardiovasculares y respiratorios de perros bajo infusión continua de dexmedetomidina. Para la selección de los estudios fueron elegidos los términos perros, dexmedetomidina e infusión continua, insertados en las plataformas Pubmed y Embase. Luego de la búsqueda, se incluyeron 11 artículos a través de criterios de inclusión y exclusión preestablecidos ya partir de estos se realizó el metanálisis. Los artículos seleccionados utilizaron dexmedetomidina en dosis que oscilaron entre 0,1-4,5 µg/kg y diferentes asociaciones. La reducción de anestésicos generales con la administración de dexmedetomidina sola fue del 11% al 69% y cuando se asoció con otros fármacos redujo la anestésica general en un 86%. En el sistema cardiovascular, la dexmedetomidina aumentó la presión arterial media y disminuyó la frecuencia cardíaca de forma dependiente de la dosis (P < 0,00). También hubo una disminución en el índice y el gasto cardíaco (P < 0,00). En cuanto a los parámetros respiratorios, no hubo diferencias significativas en la frecuencia respiratoria, dióxido de carbono y oxígeno en sangre arterial. Se concluye que la dexmedetomidina disminuye el consumo de anestésicos generales, aumenta la presión arterial media y disminuye la frecuencia cardíaca de forma dosis-dependiente, además de disminuir el gasto y el índice cardíaco, sin alterar significativamente la función respiratoria.A dexmedetomidina é um fármaco amplamente utilizado na veterinária pelos seus efeitos analgésicos, sedativos e relaxante muscular, podendo ser administrada por via intravenosa em infusão contínua com ou sem bolus inicial, porém existem alterações cardiovasculares e respiratórias que devem ser consideradas. O objetivo desta revisão foi analisar os efeitos cardiovasculares e respiratórios de cães sob infusão contínua de dexmedetomidina. Para seleção dos estudos foram escolhidos os termos cães, dexmedetomidina e infusão contínua, inseridos nas plataformas Pubmed e Embase. Após a pesquisa foram incluídos 11 artigos por meio de critérios de inclusão e exclusão pré-estabelecidos e a partir destes foi realizada a meta-análise. Os artigos selecionados utilizaram dexmedetomidina em doses variando de 0,1-4,5 µg/kg e diferentes associações. A redução de anestésicos gerais com a administração de dexmedetomidina isolada foi de 11% a 69% e quando associada com outros fármacos, reduziu o anestésico geral em 86%. Sobre o sistema cardiovascular, a dexmedetomidina aumentou a pressão arterial média e diminuiu a frequência cardíaca de maneira dose dependente (P < 0,00). Houve também diminuição do débito e índice cardíaco (P < 0,00). Sobre os parâmetros respiratórios, não houveram diferenças significativas na frequência respiratória, gás carbônico e oxigênio do sangue arterial.
