General and orofacial aspects of pychnodysostosis: report of two dental surgical intercurrences

Resumo: The aim of this article is to discuss the clinical, diagnostic and treatment aspects of dental surgical complications in patients with pychnodysostosis, through two clinical cases. Female patient, with physical characteristics of pychnodysostosis, 27 years old, complaining of pain, which appeared after undergoing a tooth extraction in the region of element 38. After clinical and radiographic examinations, a mandible fracture was found in the procedure site. The fracture was treated using a submandibular approach and fixation with a reconstruction plate, but without showing radiographic signs of reliable bone consolidation. The second case is a 23-year-old female patient with pychnodysostosis, who reported recurrent pain in the left mandibular body and frequent odors from the oral cavity, after undergoing a tooth extraction that had not healed. The treatment of the symptoms presented was performed using a delicate curettage at the site, where the presence of a cleavage plane was observed. The specimen was sent for histopathological analysis, which later showed a picture compatible with osteomyelitis with foci of bone necrosis. To date, the current literature does not present a description of a detailed clinical protocol for patients with this condition. However, some precautions must be taken to prevent bone fractures, such as avoiding mechanical trauma and invasive surgical approaches, in addition to maintaining regular dental follow-up.

Paciente de sexo femenino, con características físicas de Psicodisostosis, de 27 años de edad, que consulta por dolor, que aparece luego de ser sometido a una extracción dentaria en la región del elemento 38. Luego de exámenes clínicos y radiográficos, se encontró una fractura de mandíbula en el sitio del procedimiento. La fractura se trató mediante abordaje submandibular y fijación con placa de reconstrucción, pero sin mostrar signos radiográficos de consolidación ósea fiable. El segundo caso es una paciente de 23 años con Psicodisostosis, quien refirió dolor recurrente en el cuerpo mandibular izquierdo y frecuentes olores de la cavidad bucal, luego de ser sometida a una extracción dental que no había cicatrizado. El tratamiento de los síntomas presentados se realizó mediante un delicado curetaje en el sitio, donde se observó la presencia de un plano de clivaje. La pieza fue enviada para análisis histopatológico, que posteriormente mostró un cuadro compatible con osteomielitis con focos de necrosis ósea. Hasta la fecha, la literatura actual no presenta una descripción de un protocolo clínico detallado para pacientes con esta condición. Sin embargo, se deben tomar algunas precauciones para prevenir fracturas óseas, como evitar traumatismos mecánicos y abordajes quirúrgicos invasivos, además de mantener un seguimiento dental regular.O objetivo deste artigo é discutir os aspectos clínicos, diagnósticos e de tratamento de intercorrências cirúrgicas odontológicas em pacientes portadores de Picnodisostose, através de dois casos clínicos. Paciente do gênero feminino, com características físicas de Picnodisostose, 27 anos de idade, com queixa de dor, que surgiu após ter sido submetida a uma extração dentária na região do elemento 38. Após exames clínicos e radiográficos, foi constatada uma fratura de mandíbula no local do procedimento. A fratura foi tratada através de abordagem submandibular e fixação com placa de reconstrução, porém sem exibir sinais radiográficos de consolidação óssea confiável. O segundo caso é de uma paciente também do gênero feminino, portadora da Picnodisostose, 23 anos de idade, que referia dor recorrente em corpo mandibular esquerdo e odores frequentes provenientes da cavidade oral, após ter sido submetida a uma extração dentária que não cicatrizara. O tratamento da sintomatologia apresentada, foi realizado a partir de uma curetagem delicada no local, onde observou-se presença de plano de clivagem. O espécime foi enviado para análise histopatológica que posteriormente apontou um quadro compatível com osteomielite com focos de necrose óssea. Até o momento a literatura atual não apresenta descrição de um protocolo clínico detalhado para portadores dessa condição.
