Prevalence of comorbidities in Severe Acute Respiratory Syndrome in patients with COVID-19 and other infectious agentes

Resumo: This study aimed to analyze the cases of acute respiratory syndrome - SARS caused by COVID-19, or not, in relation to the presence of heart diseases and diabetes, in severe cases in the State of Pernambuco, the Northeast region and in Brazil. Data were collected from the Ministry of Health's OpenDataSUS Database, covering information between 12/29/2019 and 9/14/2020. Analyzes were made with the rpart package of the R software, which builds classification or regression models that can be represented as binary trees. The results revealed a significant percentage of heart diseases and diabetes (average of 51% and 40%, respectively) present in patients with SARS, regardless of etiology, in the three areas considered. It was also observed that patients with heart disease develop SARS more frequently when affected with COVID-19 (64.6%) than with other etiologies (35.4%). The same was observed for diabetic patients with COVID-19 (67.1%) and for patients negative for COVID-19 (27.7%). The lethality rate was higher among patients with both comorbidities. It was 47.6% for diabetes; 45.3% for cardiac patients and 50.3% when in combination in patients affected by SARS caused by COVID-19. The results obtained are relevant to the development of public health measures seeking to identify patients at higher risk and to apply early and specific treatments in order to reduce mortality in these cases.

Los datos se obtuvieron de la base de datos OpenDataSUS del Ministério da Saúde, que contempla información entre el 29/12/2019 y el 14/9/2020. Los análisis se realizaron con el paquete Rpart del software R, que construye modelos de clasificación o regresión que se pueden representar como árboles binarios. Los resultados revelaron un porcentaje significativo de enfermedades cardíacas y diabetes (promedio de 51% y 40%, respectivamente) presentes en pacientes con SARS, independientemente de la etiología, en las tres áreas consideradas. También se observó que los pacientes con cardiopatía desarrollan SRAG con mayor frecuencia cuando se ven afectados por COVID-19 (64,6%) que con otras etiologías (35,4%). Lo mismo se observó para los pacientes diabéticos con COVID-19 (67,1%) y para los pacientes negativos para COVID-19 (27,7%). La tasa de letalidad fue mayor entre los pacientes con ambas comorbilidades, aisladas (47,6% para diabetes y 45,3% para cardíacas) o en combinación (50,3%), afectados por SARS causado por COVID-19. Los resultados obtenidos son relevantes para el desarrollo de medidas de salud pública orientadas a identificar a los pacientes de mayor riesgo y desarrollar tratamientos precoces y específicos para reducir la mortalidad en estos casos.Este trabalho teve como objetivo analisar a associação entre confirmação ou não da Covid-19, comorbidades e casos graves da síndrome respiratória aguda - SRAG em Pernambuco-Nordeste-Brasil. Os dados foram coletados do Banco de Dados Open Data SUS do Ministério da Saúde, com informações do período 29/12/2019 e 14/09/2020. As análises foram feitas com o pacote rpart do software R, o qual constrói modelos de classificação ou regressão que podem ser representados como árvores binárias. Os resultados revelaram percentual significativo de cardiopatias e diabetes (média de 51% e 40%, respectivamente) presentes em pacientes com quadro de SRAG, independente da etiologia, nas três áreas consideradas. Observou-se, também, que pacientes cardiopatas desenvolvem SRAG com maior frequência quando acometidos com COVID-19 (64,6%) do que com outras etiologias (35,4%). O mesmo foi observado para pacientes diabéticos com COVID-19 (67,1%) e para pacientes negativos para COVID-19 (27,7%). A taxa de letalidade apresentou-se maior entre pacientes com as duas comorbidades, isoladas (47,6% para diabetes e 45,3% para cardíacos) ou em associação (50,3%), acometidos por SRAG causados por COVID-19.
