Patient with focal glioma exceeds life expectancy: case report

Resumo: Brain stem tumors are not just a single type of tumor, in fact, they are a variety of neoplasms of glial origin (gliomas). They are usually diagnosed in children aged 5 to 9 years. They may have a focal aspect (less than 2 cm) or a diffuse aspect (an infiltrative pattern). The survival of patients with focal gliomas is higher than that of patients with diffuse gliomas and, even if it is a low-grade glioma, life expectancy is a maximum of 15 years. The purpose of this report is to present the case of a six-year-old female patient with a focal brainstem tumor, located in the medulla oblongata, who had a life expectancy beyond those previously reported in the literature. It is not common to find patients whose survival exceeds expectations like the case reported. At the age of six, this patient diagnosed with a focal brainstem glioma located in the medulla oblongata, underwent treatment with chemoradiotherapy and survives. Currently she is 25 years old, with no neurocognitive disorders and with only a few motor alterations in the right side.

Suelen diagnosticarse en niños de 5 a 9 años. Pueden presentarse con un aspecto focal (menos de 2 cm) o un aspecto difuso (un patrón infiltrante). La supervivencia de los pacientes con gliomas focales es mayor que la de los pacientes con gliomas difusos y, aunque se trate de un glioma de bajo grado, la esperanza de vida es de un máximo de 15 años. El objectivo del presente reporte es presentar el caso de una paciente del sexo femenino de seis años de edad con tumor de tronco encefálico, con localización focal en el bulbo raquídeo, quien tuvo una expectativa de vida superior a las reportadas previamente en la literatura. No es común encontrar pacientes cuya supervivencia supere las expectativas como el caso presentado. A la edad de seis años, esta paciente, diagnosticada con un glioma de tronco encefálico de localización focal en el bulbo raquídeo, se sometió a tratamiento con quimiorradioterapia y sobrevive. Actualmente tiene 25 años, sin alteraciones neurocognitivas y solo con algunas alteraciones motoras en el lado derecho.Os tumores de tronco cerebral não são apenas um único tipo de tumor, na verdade, trata-se de uma variedade de neoplasias de origem glial (gliomas). Geralmente são diagnosticados em crianças numa faixa etária de 5 a 9 anos. Podem se apresentar com aspecto focal (menor de 2 cm) ou aspecto difuso (um padrão infiltrativo). A sobrevida de portadores de gliomas focais é maior que a dos pacientes com gliomas difusos e ainda que seja uma glioma de baixo grau, a expectativa de vida é de no máximo 15 anos. O objetivo do presente relato é apresentar o caso de uma paciente de seis anos com tumor no tronco cerebral, com localização focal no bulbo que teve uma expectativa de vida além das reportadas anteriormente na literatura. Não é comum encontrar pacientes cuja sobrevida supera as expectativas como o caso relatado. Aos seis anos esta paciente diagnosticada com um glioma de tronco cerebral de localização focal no bulbo, foi submetida ao tratamento com quimioradioterapia e sobrevive.
