Epidemiological profile of patients with non-traumatic respiratory complications in the urgency and emergency unit of a University Hospital in Paraná and correlation of physical therapy performance

Resumo: Respiratory system disorders affect all age groups and can generate severity, morbidity and mortality. The urgency and emergency service is the patient's gateway to the hospital, with functionality 24 hours a day, offering a multiprofessional and specialized approach to patients who can rapidly evolve to life-threatening and serious complications. The physical therapist plays a fundamental role, directly intervening in cardiorespiratory disorders, considering that pulmonary alterations are one of the main reasons for hospital admissions. The objective of the present study was to carry out a data collection of patients admitted to the emergency room of the Hospital Universitário do Oeste do Paraná (HUOP) with signs, symptoms and diagnosis of diseases associated with the respiratory system and the respective physiotherapeutic performance. 1020 medical records of patients in the emergency unit were analyzed in 2020, where 89 patients met the inclusion criteria. There was a greater predominance in females (n=47, 52.8%). The main cause of hospitalization was acute respiratory failure (n=36, 40.4%). Pulmonary sepsis was the pathology with the longest stay in the emergency room (5 days). The physiotherapeutic approach was performed in the vast majority of cases 76 (85.3%), where 48 (63%) patients were approached with respiratory physiotherapy, motor physiotherapy and airway aspiration. It is concluded that patients with respiratory complications such as acute respiratory failure and sepsis have a high mortality rate and physical therapy is important in the process of rehabilitation and ventilatory management of these patients.

El servicio de urgencias y emergencias es la puerta de entrada del paciente al hospital, con funcionalidad las 24 horas del día, ofreciendo un abordaje multiprofesional y especializado a pacientes que pueden evolucionar rápidamente hacia complicaciones graves y potencialmente mortales. El fisioterapeuta juega un papel fundamental, interviniendo directamente en los trastornos cardiorrespiratorios, considerando que las alteraciones pulmonares son uno de los principales motivos de ingreso hospitalario. El objetivo del presente estudio fue realizar una recolección de datos de pacientes ingresados ​​en el servicio de urgencias del Hospital Universitário do Oeste do Paraná (HUOP) con signos, síntomas y diagnóstico de enfermedades asociadas al sistema respiratorio y la respectiva actuación fisioterapéutica. En el año 2020 se analizaron 1020 historias clínicas de pacientes en la unidad de emergencia, donde 89 pacientes cumplieron con los criterios de inclusión. Hubo mayor predominio en el sexo femenino (n=47, 52,8%). La principal causa de hospitalización fue la insuficiencia respiratoria aguda (n=36, 40,4%). La sepsis pulmonar fue la patología con mayor estancia en urgencias (5 días). El abordaje fisioterapéutico se realizó en la gran mayoría de los casos 76 (85,3%), donde 48 (63%) pacientes fueron abordados con fisioterapia respiratoria, fisioterapia motora y aspiración de vías aéreas. Se concluye que los pacientes con complicaciones respiratorias como insuficiencia respiratoria aguda y sepsis tienen una alta tasa de mortalidad y la fisioterapia es importante en el proceso de rehabilitación y manejo ventilatorio de estos pacientes.Os distúrbios do aparelho respiratório acometem todas as faixas etárias, podendo gerar severidade, morbidade e mortalidade. O objetivo do presente estudo foi realizar um levantamento de dados de pacientes admitidos na sala de emergência do Hospital Universitário do Oeste do Paraná (HUOP) com sinais, sintomas e diagnóstico de enfermidades associadas ao aparelho respiratório e a respectiva atuação fisioterapêutica. Foram analisados 1020 prontuários de pacientes na unidade de emergência no ano de 2020, onde 89 pacientes se enquadraram nos critérios de inclusão. Houve uma maior predominância no sexo feminino (n=47, 52.8%). A principal causa de internação foi a insuficiência respiratória aguda (n=36, 40.4%). A sepse pulmonar foi a patologia com maior tempo de permanência em sala de emergência (5 dias). A abordagem fisioterapêutica foi realizada na grande maioria dos casos 76 (85.3%), onde 48(63%) pacientes foram abordados com condutas de fisioterapia respiratória, fisioterapia motora e aspiração de vias aéreas.
