Pleomorphic adenoma: Case report

Resumo: Pleomorphic adenoma is a benign tumor, common in salivary glands, affecting both major and minor glands. When it occurs in the minor salivary glands, the palate is the main site affected. Clinically, it presents as a painless, slow-growing, encapsulated volumetric increase. In histopathological terms, this tumor is considered mixed, since it is composed of myoepithelial and epithelial cells and has its cells separated from the rest of the tissues by means of a fibrous capsule. Treatment is by surgical excision with wide margins. This article aims to report a clinical case of a patient with a pleomorphic adenoma in the minor salivary gland located on the palate. Management was presented until obtaining its definitive diagnosis, as well as the form of treatment, seeking clinical alternatives that would provide a postoperative period with less painful symptoms. After 3 months, the patient was followed up and no clinical signs of recurrence were observed until that moment. Considering that the pleomorphic adenoma can undergo malignant transformation into a former pleomorphic adenoma, it is of paramount importance that the diagnosis is properly established, the best treatment instituted and periodic observations are made.

Cuando se presenta en las glándulas salivales menores, el paladar es el principal sitio afectado. Clínicamente se presenta como un aumento volumétrico encapsulado, indoloro y de crecimiento lento. En términos histopatológicos, este tumor se considera mixto, ya que está compuesto por células mioepiteliales y epiteliales y tiene sus células separadas del resto de los tejidos por medio de una cápsula fibrosa. El tratamiento es mediante escisión quirúrgica con márgenes amplios. Este artículo tiene como objetivo reportar un caso clínico de un paciente con un adenoma pleomórfico en una glándula salival menor ubicada en el paladar. Se presentó el manejo hasta obtener su diagnóstico definitivo, así como la forma de tratamiento, buscando alternativas clínicas que brindaran un postoperatorio con síntomas menos dolorosos. A los 3 meses se realizó el seguimiento de la paciente y hasta ese momento no se observaron signos clínicos de recidiva. Teniendo en cuenta que el adenoma pleomórfico puede sufrir una transformación maligna en un antiguo adenoma pleomórfico, es de suma importancia que el diagnóstico se establezca correctamente, se instituya el mejor tratamiento y se realicen observaciones periódicas.O adenoma pleomórfico é um tumor benigno, comum em glândulas salivares acometendo tanto as glânculas maiores quanto as menores. Quando ocorre nas glândulas salivares menores, o palato é o principal local afetado. Clinicamente apresenta-se como um aumento volumétrico indolor, de crescimento lento e encapsulado. Em termos histopatológicos, este tumor é considerado misto, uma vez que é composto por células mioepiteliais e epiteliais e tem suas células separadas do restante dos tecidos por meio de uma cápsula fibrosa. O tratamento é por excisão cirúrgica com margens amplas. Este artigo tem como objetivo realizar o relato de um caso clínico de uma paciente portadora de um adenoma pleomórfico em glândula salivar menor localizado em palato. Foi apresentado o manejo até obtenção do seu diagnóstico definitivo, bem como a forma de tratamento, buscando alternativas clínicas que propiciassem um pós operatório com menor sintomatologia dolorosa. Após 3 meses, foi realizada a proservação da paciente e não foram observados indícios clínicos de recorrência até aquele presente momento.
