Updates on the influence of obesity on the onset of gestational diabetes and its complications for mother and fetus

Resumo: | Variations in carbohydrate metabolism are evident during pregnancy to meet the needs of the fetus. However, when pregnant women present exacerbated physiological alterations of this mechanism associated with maternal predisposition, Gestational Diabetes Mellitus (GDM) is characterized. Pregnant women who have risk factors such as insulin resistance and overweight/obesity are more likely to develop the disease, favoring adverse outcomes for the fetus. Thus, the present study aims to find evidence in the literature of the relationship between overweight/obesity and the development of GDM, given the epidemiology of the disease and its higher prevalence as one of the main complications of the gestational period. The article consists of an integrative literature review, with a publication interval between 2020 and 2022. The following databases were used: Virtual Health Library (VHL); National Library of Medicine (PubMed MEDLINE), Scientific Electronic Library Online (SCIELO), EbscoHost. The results demonstrated a positive relationship between overweight/obesity and GDM, demonstrating that the complications of this relationship affect maternal and fetal health, and may progress to unfavorable outcomes in the infant and adult life of the conceptus. Therefore, the conclusion of this article is in agreement with recent findings in the literature, which show that obesity, when present in pregnant women, is directly related to several disorders both in the mother, such as the development of GDM, and in the fetus, increasing intrauterine and neonatal morbidity and mortality. |

Sin embargo, cuando las gestantes presentan alteraciones fisiológicas exacerbadas de este mecanismo, se caracteriza la Diabetes Mellitus Gestacional (DMG). Las mujeres embarazadas que presentan factores de riesgo como resistencia a la insulina y sobrepeso/obesidad tienen mayor probabilidad de desarrollar la enfermedad, favoreciendo desenlaces adversos para el feto. Así, el presente estudio tiene como objetivo encontrar evidencias en la literatura de la relación entre el sobrepeso/obesidad y el desarrollo de DMG, dada la epidemiología de la enfermedad y su mayor prevalencia como una de las principales complicaciones del período gestacional. El artículo consiste en una revisión integrativa de la literatura, con intervalo de publicación entre 2020 y 2022. Se utilizaron las siguientes bases de datos: Biblioteca Virtual en Salud (BVS); Biblioteca Nacional de Medicina (PubMed MEDLINE), Biblioteca Científica Electrónica en Línea (SCIELO), EbscoHost. Los resultados demostraron una relación positiva entre sobrepeso/obesidad y DMG, demostrando que las complicaciones de esta relación afectan la salud materna y fetal, y pueden progresar a resultados desfavorables en la vida infantil y adulta del feto. Por lo tanto, la conclusión de este artículo está de acuerdo con hallazgos recientes en la literatura, que muestran que la obesidad, cuando está presente en mujeres embarazadas, está directamente relacionada con varios trastornos tanto en la madre, como el desarrollo de DMG, como en feto, aumento de la morbilidad y mortalidad intrauterina y neonatal.As variações no metabolismo dos carboidratos são evidenciadas durante a gravidez para suprir as necessidades do feto. Entretanto, quando as gestantes apresentam alterações fisiológicas exacerbadas desse mecanismo associadas a predisposição materna, caracteriza-se o Diabetes Mellitus Gestacional (DMG). As gestantes que apresentam fatores de risco como resistência à insulina e sobrepeso/obesidade, apresentam maior probabilidade que a doença se desenvolva, favorecendo desfechos adversos para o feto. Assim, o presente estudo tem por objetivo encontrar na literatura evidências da relação entre sobrepeso/obesidade e o desenvolvimento de DMG, dada a epidemiologia da doença e sua maior prevalência como uma das principais complicações do período gestacional. O artigo consiste em uma revisão integrativa de literatura, com intervalo de publicação entre 2020 e 2022.
