Use of aromatase inhibitors for height improvement in boys: a literature review

Resumo: | Introduction: Short stature is currently one of the most common reasons for seeking and referring patients to pediatric endocrinology. Such a demand occurs not only due to the physiological factors, but also the psychological ones involved. Objective: to analyze the specific literature on aromatase inhibitors and their use in children. Method: A narrative literature review was carried out using Pubmed, Google Scholar and UptoDate databases. For the search, the following descriptors were used: “aromatase inhibitor”, “children” and “growth”. Articles on the use of aromatase inhibitors in boys with short stature were selected. Articles that focused on the use of aromatase inhibitors in cancer, the use of aromatase inhibitors exclusively in McCune Albright Syndrome and that were not located in full were excluded. A total of 1028 scientific productions were found, of which 59 met the pre-established criteria. Results and Discussion: The aromatase enzyme is a cytochrome P450 enzyme and converts androgenic precursors into estrogenic precursors. Aromatase inhibitors delay growth cartilage senescence and may be indicated in boys with current short stature with psychosocial repercussions or inadequate prediction of final height. According to previously published studies, aromatase inhibitors have been shown to be effective in the treatment of short stature. It is concluded that aromatase inhibitor treatment effectively delays bone maturation and improves adult height. |

Tal demanda se produce no sólo por los factores fisiológicos, sino también psicológicos involucrados. Objetivo: analizar la literatura específica sobre los inhibidores de la aromatasa y su uso en niños. Método: Se realizó una revisión narrativa de la literatura utilizando las bases de datos Pubmed, Google Scholar y UptoDate. Para la búsqueda se utilizaron los siguientes descriptores: “inhibidor de la aromatasa”, “niños” y “crecimiento”. Se seleccionaron artículos sobre el uso de inhibidores de la aromatasa en niños con talla baja. Se excluyeron los artículos que se enfocaron en el uso de inhibidores de aromatasa en el cáncer, el uso de inhibidores de aromatasa exclusivamente en el Síndrome de McCune Albright y que no fueron localizados en su totalidad. Se encontraron un total de 1028 producciones científicas, de las cuales 59 cumplieron con los criterios preestablecidos. Resultados y Discusión: La enzima aromatasa es una enzima del citocromo P450 y convierte precursores androgénicos en precursores estrogénicos. Los inhibidores de la aromatasa retrasan la senescencia del cartílago de crecimiento, y pueden estar indicados en niños con talla baja actual con repercusiones psicosociales o predicción inadecuada de la talla final. Según estudios publicados anteriormente, los inhibidores de la aromatasa han demostrado ser efectivos en el tratamiento de la baja estatura. Se concluye que el tratamiento con inhibidores de la aromatasa retrasa efectivamente la maduración ósea y mejora la talla adulta.Introdução: Baixa estatura, atualmente, é uma das causas mais comuns de procura e encaminhamento de pacientes para a endocrinologia pediátrica. Tal procura se dá, não somente devido aos fatores fisiológicos, mas também psicológicos envolvidos. Objetivo: analisar a literatura específica sobre os inibidores da aromatase e seu uso em crianças. Método: Foi realizada uma revisão narrativa de literatura, utilizando as bases de dados Pubmed, Google acadêmico e UptoDate. Para a busca, foram utilizados os descritores: “aromatase inibithor”, “children” e “growth”. Foram selecionados artigos sobre o uso dos inibidores da aromatase em meninos com baixa estatura. Excluíram-se artigos que se voltavam para uso de inibidores da aromatase em câncer, uso dos inibidores da aromatase exclusivamente em Síndrome McCune Albright e que não estavam localizados na íntegra. Foram encontradas 1028 produções cientificas, das quais 59 correspondiam aos critérios pré-estabelecidos. Resultados e Discussão: A enzima aromatase é uma enzima do citocromo P450 e faz a conversão de precursores androgênicos em precursores estrogênicos. Os inibidores da aromatase retardam a senescência da cartilagem de crescimento, podem ser indicados em meninos com baixa estatura atual com repercussão psicossocial ou previsão inadequada da estatura final. Segundo estudos previamente publicados, os inibidores da aromatase foram eficazes no tratamento de baixa estatura.
