Risk factors for suicide in the elderly before and during the COVID-19 confinement period

Resumo: Analyzing the different aspects related to the mental health and risk factors for elderly suicide and verify its association with COVID-19 pandemic. This is an integrative literature review carried out by a search for scientific articles indexed in electronic databases available on Virtual Health Library (VHL) and SciELO. The keywords used were: “mental health” OR “suicide” AND “elderly” OR “aged” AND “pandemics” AND “covid-19” OR “coronavirus”, and included 33 articles. The most associated factors with the suicide of brazilian elders were: health problems, isolation, violence, financial problems and loss of loved ones. Brazilian studies that addressed biopsychosocial aspects of suicide used, mostly, semi-structured interviews as methodological path. Studies that address the mental health of the elderly in the midst of the pandemic have European, North American and Asian origins, with varied methodologies. These showed that covid-19 isolation comprises risk factors for suicide among the elderly. Although some studies point to the resilience of the elderly during the pandemic, the research in general has explained the vulnerability to the risks to mental health. Therefore, the COVID-19 pandemic can bring consequences to the health of the population, reflecting also on mental health – due to the multiplicity of associated losses, especially among the elderly. Regarding the pre-pandemic studies, it was noted that the confinement for covid-19 presents challenges and risk factors for suicide among this population. In Brazil, public policies aimed at preventing suicide among the elderly are still not a priority, making it difficult to identify risk factors for suicidal ideations.

Se trata de una revisión integrativa de literatura mediante la búsqueda de artículos indexados por bases de datos electrónicas de Biblioteca Virtual en Salud (BVS) y SciELO, incluyendo 33 artículos.  Los descriptores fueron: “mental health” OR “suicide” AND “elderly” OR “aged” AND “pandemics” AND “covid-19” OR “coronavirus”. Los factores más asociados con el suicidio de ancianos en Brasil fueron: problemas de salud, aislamiento, violencia, problemas económicos y pérdida de seres queridos. Los estudios brasileños utilizaron, en su mayoría, entrevistas semiestructuradas como camino metodológico. Los estudios que abordan la salud mental de ancianos en la pandemia tienen orígenes europeos, norteamericanos y asiáticos, con metodologías variadas. Estos mostraron que el aislamiento comprende factores de riesgo de suicidio entre las personas mayores. Aunque algunos estudios apuntan la resiliencia de los ancianos en la pandemia, los estudios en general han explicado la vulnerabilidad a riesgos para salud mental. Por consiguiente, la pandemia de COVID-19 puede tener consecuencias para la salud de la población, afectando también la salud mental, debido a la multiplicidad de pérdidas asociadas, especialmente entre ancianos. En relación con los estudios prepandémicos, se observó que el confinamiento por covid-19 presenta desafíos y factores de riesgo de suicidio entre la población anciana. En Brasil, las políticas públicas destinadas a prevenir el suicidio entre personas mayores aún no son una prioridad, realidad que dificulta la identificación de riesgos para las ideas suicidas.Analisar os fatores relacionados à saúde mental e de risco para suicídio entre idosos e verificar sua associação com a pandemia de COVID-19. Trata-se de uma revisão integrativa da literatura a partir da busca por artigos das bases eletrônicas das plataformas Biblioteca Virtual em Saúde (BVS) SciELO. Os termos de pesquisa foram: “mental health” OR “suicide” AND “elderly” OR “aged” AND “pandemics” AND “covid-19” OR “coronavirus”, incluindo 33 artigos. Os fatores mais associados ao suicídio de idosos no Brasil foram: problemas de saúde, isolamento, violência, problemas financeiros e perda de entes queridos. Os estudos brasileiros utilizaram, em maioria, entrevistas semiestruturadas como percurso metodológico. Os estudos que abordam a saúde mental dos idosos na pandemia possuem origem europeia, norte-americana e asiática, com metodologias variadas. Estes mostraram que o isolamento por covid-19 compreende os fatores de risco para o suicídio entre idosos. Embora trabalhos apontem a resiliência dos idosos em meio à pandemia, os estudos em geral explicitaram a vulnerabilidade frente aos riscos à saúde mental. Assim, a pandemia de COVID-19 pode trazer consequências à saúde da população, repercutindo, também, na saúde mental – devido à multiplicidade de perdas associadas, especialmente entre os idosos. Em relação aos estudos pré-pandêmicos, notou-se que o confinamento por covid-19 apresenta desafios e fatores de risco para o suicídio entre idosos.
