Treatment of Post-traumatic Lipoma on the face: case report

Resumo: Lipoma is a benign mesenchymal tumor of tissue that appears in places with the presence of adipocytes. They are more common in adulthood, being found in 80% of the cases in the chest and in the extremities of the body. Less frequently, about 13-20%, manifest in the head and neck. The tumor shows a predilection for Caucasian males, between 49 and 60 years old. The lipomas have anetiology related to genetic diseases, endocrine disorders, heredity, local infection and trauma. The treatment of choice is surgical excision of the lesion, with rare cases of recurrence. Generally, these are associated with in adequate resection of neoplasia, especially, of intramuscular lipomas with an infiltrative pattern. Thus, the objective of this article is to present a clinical case of lipoma  that developed after suffering trauma to the face region. A case of a 53 year old male patient, feoderma, with a motorcycle accident about 3 years was described. On physical examination, there was an in crease in the circumscribed volume in the right temporofrontal region, the same region affected 3 years ago in the previous accident, with about 10 cm in diameter, softened palpation, normal coloration of the adjacent tissue, with out related pain complaints. A fine needle aspiration biopsies (FNAB) was performed, which was negative and, subsequently, surgical excision of the lesion. After excision al biopsy, it was confirmed by macroscopic diagnosis, which was adipose tissue, compatible with a lipoma. The patient continues two months after the operation, with no signs of recurrence.

Son más comunes en la edad adulta, y se encuentran en el 80% de los casos en el tórax y las extremidades del cuerpo. Con menos frecuencia, alrededor del 13-20%, se manifiestan en la cabeza y el cuello. El tumor muestra una predilección por los varones caucásicos de entre 49 y 60 años. Los lipomas tienen una etiología relacionada con enfermedades genéticas, trastornos endocrinos, herencia, infección local y trauma. El tratamiento de elección es la escisión quirúrgica de la lesión, con casos raros de recurrencia. En general, estos están asociados con una resección inadecuada de la neoplasia, especialmente de los lipomas intramusculares con un patrón infiltrativo. Por lo tanto, el objetivo de este artículo es presentar un caso clínico de lipoma que se desarrolló después de sufrir un trauma en la región de la cara. Se describió un caso de un paciente masculino de 53 años, feoderma, con un historial de accidentes de motocicleta de aproximadamente 3 años. En el examen físico, hubo un aumento en el volumen circunscrito en la región temporofrontal derecha, la misma región afectada hace 3 años en el accidente anterior, con aproximadamente 10 cm de diámetro, palpación suavizada, coloración normal del tejido adyacente, sin quejas de dolor relacionadas. Se realizó una punción por aspiración con aguja fina (PAAF), que fue negativa y, posteriormente, escisión quirúrgica de la lesión. Después de la biopsia excisional, se confirmó mediante diagnóstico macroscópico, que era tejido adiposo, compatible con un lipoma. El paciente continúa dos meses después de la operación, sin signos de recurrencia.O Lipoma é um tumor mesenquimal benigno de tecido mole que surge em locais com presença de adipócitos. São mais comuns na vida adulta, sendo encontrados em 80% dos casos no tórax e nas extremidades do corpo. Em menor freqüência, cerca de 13-20%, manifestam-se em cabeça e pescoço. O tumor exibe predileção por caucasianos do sexo masculino, entre 49 e 60 anos. Os lipomas detêm uma etiologia relacionada a doenças genéticas, distúrbios endócrinos, hereditariedade, infecção local e trauma. O tratamento de escolha é a excisão cirúrgica da lesão, sendo raros casos de recorrência. Geralmente, estes estão associados á ressecção inadequada da neoplasia, sobretudo, de lipomas intramusculares com padrão infiltrativo. Assim o objetivo deste artigo é apresentar  um caso clínico de lipoma que se desenvolveu após um trauma em região de face. Foi descrito um caso de uma paciente do sexo masculino, 53 anos, feoderma, com histórico de acidente motociclistico há cerca de 3 anos. Ao exame físico observou-se aumento de volume circunscrito em região temporofrontal direita, mesma região acometida há 3 anos no acidente pregresso, com cerca de 10 cm de diâmetro, amolecida a palpação, coloração normal do tecido adjacente, sem queixas álgicas relacionadas. Foi realizada uma punção aspirativa de agulha fina (PAAF), o qual apresentou-se negativa, em seguida excisão cirúrgica da lesão. Após biopsia excisional, foi confirmado por meio de diagnóstico macroscópico, que se tratava de tecido adiposo, compatível com um lipoma.
