Hospital readmission in patients submitted to heart surgery: prospective cohort

Resumo: Hospital readmissions are critical phenomena that delay the return of patients to their activities that maintain life, health and well-being. Objective:To assess the incidence and causes of readmission after hospital discharge in patients undergoing cardiac surgery. Method:Prospective observational study. The study included 181 patients who underwent cardiac surgery and were observed in the immediate preoperative periods and after hospital discharge on the 7th, 15th and 30th, through telephone contacts.Results:Of the 181 participants included in the study, 13 were discontinued due to intrahospital death and 1 due to transfer to another institution. Therefore, telephone contact was made with 167 patients, among whom 16 (9.58%) were readmitted. Of these, 2 were readmitted 7 days after hospital discharge; 7 were readmitted 15 days after discharge; and 7 readmitted 30 days after hospital discharge, with most readmissions due to infection of the surgical site in the mediastinum (18.75%) and saphenous vein (18.75%). Conclusion:Most readmissions were due to infection of the surgical site. This means that strategies to control and minimize these events should be part of the follow-up of cardiac surgery patients.

El estudio incluyó a 181 pacientes que se sometieron a cirugía cardíaca y fueron observados en los períodos preoperatorios inmediatos y después del alta hospitalaria los días 7, 15 y 30, por medio de contactos telefónicos. Resultados:De los 181 participantes incluidos en el estudio, 13 fueron descontinuados debido a muerte en el hospital y 1 debido a la transferencia a otra institución. Por lo tanto, se estableció contacto telefónico con 167 pacientes, de los cuales 16 (9,58%) fueron readmitidos. De estos, 2 fueron readmitidos 7 días después del alta hospitalaria; 7 fueron readmitidos 15 días después del alta; y 7 readmitidos 30 días después del alta hospitalaria, con la mayoría de los reingresos debido a infección del sitio quirúrgico en el mediastino (18,75%) y la vena safena (18,75%). Conclusión:la mayoría de los reingresos se debieron a infección del sitio quirúrgico. Esto significa que las estrategias para controlar y minimizar estos eventos deberían ser parte del seguimiento de los pacientes de cirugía cardíaca.As readmissões hospitalares são fenômenos críticos, que retardam o retorno dos pacientes às suas atividades que mantêm a vida, a saúde e o bem-estar. Objetivo: Avaliar a incidência e as causas de readmissão após a alta hospitalar em pacientes submetidos a cirurgia cardíaca. Método: Estudo observacional prospectivo. Foram incluídos no estudo 181 pacientes submetidos à cirurgia cardíaca e observados nos períodos pré-operatório imediato e após alta hospitalar nos dias 7, 15 e 30, por meio de contatos telefônicos. Resultados: Dos 181 participantes incluídos no estudo, 13 foram descontinuados por óbito intrahospitalar e 1 por transferência para outra instituição. Portanto, foram realizados contato telefônico com 167 pacientes, dentre os quais 16 (9,58%) foram readmitidos.  Destes, 2 foram readmitidos 7 dias após a alta hospitalar; 7 foram readmitidos 15 dias após a alta; e 7 readmitidos 30 dias após a alta hospitalar, sendo que, em sua maioria, as readmissões foram por infecção de sítio cirúrgico em mediastino (18,75%) e safena (18,75%). Conclusão: A maior parte das readmissões foi por infeção de sítio cirúrgico.
