The effectiveness of chiropractic vertebral manipulation for the treatment of migraine: a systematic review

Resumo: The present study, registered with PROSPERO under the number CRD42022321874. Aims: Discuss current randomized clinical trials in the literature to investigate the effectiveness of chiropractic spinal manipulation in the treatment of migraine. Methodology: The search was carried out during the period from February to May 2022 through the databases: PubMed, Scielo, PEDro and VHL, limited to articles identified as randomized clinical trials (RCTs), in any language, which used as primary intervention chiropractic spinal manipulation and migraine as a primary disorder investigated. The search terms included: Headache, migraine, manipulation, chiropractic. Results: Our search identified 4 RCTs that met all inclusion criteria. The methodological quality was mostly low and ranged between 4 and 8 on the PEDro scale. Interventions lasted from 2 to 6 months and outcome averages included migraine days, pain, migraine intensity and disability. 3 RCTs showed improvements compared to the control group in migraine days, intensity, duration and disability. 1 RCT did not obtain relevant results in combining spinal manipulation with the use of amitriptyline. Conclusion: However, it was not clear from the results whether chiropractic spinal manipulation can be effective in the treatment of migraine, so it suggests that other much better elaborated studies be added in the literature, before stating whether this treatment can be effective or not for pain.

Objetivo: Discutir los ensayos clínicos aleatorios actuales en la literatura para investigar la efectividad de la manipulación espinal quiropráctica en el tratamiento de la migraña. Metodología: La búsqueda se realizó durante el período de febrero a mayo de 2022 a través de las bases de datos: PubMed, Scielo, PEDro y BVS, limitada a artículos identificados como ensayos clínicos aleatorizados (ECA), en cualquier idioma, que utilizaran como intervención primaria la quiropráctica espinal. La manipulación y la migraña como un trastorno primario investigado. Los términos de búsqueda incluyeron: dolor de cabeza, migraña, manipulación, quiropráctica. Resultados: Nuestra búsqueda identificó 4 ECA que cumplieron con todos los criterios de inclusión. La calidad metodológica fue en su mayoría baja y osciló entre 4 y 8 en la escala PEDro. Las intervenciones duraron de 2 a 6 meses y los promedios de los resultados incluyeron días con migraña, dolor, intensidad de la migraña y discapacidad. 3 ECA mostraron mejoras en comparación con el grupo control en días de migraña, intensidad, duración y discapacidad. Un ECA no obtuvo resultados relevantes al combinar la manipulación espinal con el uso de amitriptilina. Conclusión: Aún, no quedó claro a través de los resultados se a manipulación vertebral quiropráctica pueda ser eficaz, no tratamiento da migraña, para eso sugiere que ostros estudios bien mejores elaborados cejan adicionados la literatura, antes de afirmar sé ese tratamiento pueda ser eficaz o no para el dolor de cabeza.O presente estudo, cadastrado na PROSPERO sob o número CRD42022321874. Objetivo: Discutir os ensaios clínicos randomizados vigentes na literatura para investigar a eficácia da manipulação vertebral quiroprática no tratamento de enxaqueca. Metodologia: A busca foi realizada durante o período de fevereiro a maio de 2022 através das bases de dados: PubMed, Scielo, PEDro e BVS, limitada a artigos identificados como ensaio clínico randomizado (ECRs), em qualquer idioma, que usaram como intervenção primaria a manipulação vertebral quiroprática e a enxaqueca como distúrbio primário investigado. Os termos incluídos na pesquisa: Headache, migrain, manipulation, chiropractic. Resultados: Nossa busca identificou 4 ECRs que atenderam a todos os critérios de inclusão. A qualidade metodológica em sua maioria foi baixa e variou entre 4 e 8 na escala PEDro. As intervenções duraram de 2 a 6 meses e as medias de resultados incluíram dias de enxaqueca, dor, intensidade e incapacidade da enxaqueca. 3 ECRs mostraram melhorias em comparação ao grupo controle nos dias de enxaqueca, intensidade, duração e incapacidade. 1 ECR não obteve resultados relevantes em combinar a manipulação vertebral com o uso de amitriptilina.
