Correlation between respiratory system compliance and Covid-19 phenotypes and clinical outcomes in an ICU in the state of Pernambuco

Resumo: Background: SARS-CoV-2 disease can lead to respiratory failure and the need for mechanical ventilation. Pneumonia caused by Covid-19 presents with 2 phenotypes that course with different pathophysiology, type 1 and type 2. Aim: The objective of the study was to correlate the phenotypic profile of patients with acute respiratory distress syndrome diagnosed with Covid-19. 19 and their outcomes. Methods: This is an analytical retrospective cross-sectional study, which used clinical data from ICU patients at Hospital das Clínicas, Universidade Federal de Pernambuco. Data were described in simple frequencies. The correlation between hospitalization and pulmonary physiology variables with clinical outcome was tested using Pearson's chi-square test. For variables with a significant association, logistic regression was performed with the outcome of ICU discharge, and the results were presented with odds ratio. Results: An association was observed between death and patients aged > 60 years (p=0.003), length of stay ≤ 11 days (p=0.001) and static compliance ≤ 35 ml/cmH2O (p=0.045). Multiple logistic regression with the high outcome found significant values for compliance ≤ 35 ml/H2O (p-value=0.029) and length of stay ≤ 11 days (p=0.014). Conclusion: in the present study, it was observed that the factors age greater than 60 years, shorter hospital stay and lower lung compliance were associated with the outcome of death in patients with Covid-19.

La neumonía por Covid-19 se presenta con 2 fenotipos que cursan con diferente fisiopatología, tipo 1 y tipo 2. Objetivo: El objetivo del estudio fue correlacionar el perfil fenotípico de pacientes con síndrome de dificultad respiratoria aguda diagnosticados de Covid-19 y sus resultados. Métodos: Se trata de un estudio transversal analítico retrospectivo, que utilizó datos clínicos de pacientes de la UCI del Hospital das Clínicas de la Universidade Federal de Pernambuco. Los datos se describieron en frecuencias simples. La correlación entre la hospitalización y las variables de fisiología pulmonar con el resultado clínico se probó mediante la razón de probabilidades (OR) y la prueba de chi-cuadrado de Pearson. Para las variables con asociación significativa se realizó una regresión logística con el desenlace alta de UCI y los resultados se presentaron con OR. Resultados: Se observó asociación entre muerte y pacientes con edad > 60 años (p=0,003), tiempo de estancia ≤ 11 días (p=0,001) y distensibilidad estática ≤ 35 ml/cmH2O (p=0,045). La regresión logística múltiple con el resultado alto encontró valores significativos para el cumplimiento ≤ 35 ml/H2O (valor de p = 0,029) y duración de la estancia ≤ 11 días (p = 0,014). Conclusión: en el presente estudio se observó que los factores edad mayor de 60 años, menor estancia hospitalaria y menor distensibilidad pulmonar se asociaron con el desenlace de muerte en pacientes con Covid-19.Introdução: A doença do SARS-CoV-2 pode levar a falência respiratória e necessidade de ventilação mecânica. Resultados: Foi observada associação entre o óbito e pacientes com idade > 60 anos (p=0,003), tempo de internamento ≤ 11 dias (p=0,001) e a complacência estática ≤ 35 ml/cmH2O (p=0,045). A regressão logística múltipla com o desfecho alta encontrou valores significativos para complacência ≤ 35 ml/H2O (p-valor=0,029) e tempo de internamento ≤ 11 dias (p= 0,014).
