Epidemiological profile of syphilis in pregnant women and congenital: historical series from 2011 to 2020 in the Metropolitan Health Region I, state of Pará, Brazil

Resumo: Objective: To describe the epidemiological profile of syphilis in pregnant women (SG) and congenital (SC) in Health Region I, Pará state, Brazil, in the historical series from 2011 to 2020. Methodology: Ecological study of mixed design that aims to analyze the temporal trend and spatial distribution of SG and SC. The data were obtained through the Panel of Epidemiological Indicators of the Ministry of Health. Results: 3038 cases of SG and 1703 cases of SC were reported. Belém had the highest frequency of SG and SC cases, followed, respectively, by Ananindeua, Marituba, Benevides and Santa Bárbara. Significant increases in the incidence of SG were observed in all municipalities in Health Region I (p<0.05). As for the incidence of SC, there was a significant increase in Belém and Ananindeua (p<0.05), but the variations in rates were not significant in Marituba (p=0.169), Benevides (p=0.632) and Santa Bárbara (p=0.731). Among pregnant women with syphilis, there was a predominance of brown women (78.5%); age group from 20 to 29 years (57.2%); intermediate (34.0%) and low (33.5%) level of education; with a record of “prenatal care” (76.2%); who were diagnosed in the 3rd trimester of pregnancy (58%); and had inadequate treatment (48.8%). Most SC diagnoses were performed in children younger than 7 days of life (95.9%). Conclusion: The increasing incidence of SG and SC in Health Region I, in the state of Pará, points to the need for improvements in actions related to the prevention, diagnosis, treatment and control of the disease, as well as in prenatal care.

Los datos fueron obtenidos através del Panel de Indicadores Epidemiológicos del Ministerio de Salud. Resultados: se reportaron 3038 casos de SG y 1703 casos de SC. Belém presentó la mayor frecuencia de casos de SG y SC, seguida, respectivamente, por Ananindeua, Marituba, Benevides y Santa Bárbara. Se observaron aumentos significativos en la incidencia de SG en todos los municipios de la Región de salud I (p<0,05). En cuanto a la incidencia de SC, hubo un aumento significativo en Belém y Ananindeua (p<0,05), pero las variaciones en las tasas no fueron significativas en Marituba (p=0,169), Benevides (p=0,632) y Santa Bárbara (p= 0.731). Entre las gestantes con sífilis, hubo predominio de morenas (78,5%); grupo de edad de 20 a 29 años (57,2%); nivel educativo medio (34,0%) y bajo (33,5%); con registro de “atención prenatal” (76,2%); que fueron diagnosticadas en el 3er trimestre del embarazo (58%); y tuvo tratamiento inadecuado (48,8%). La mayoría de los diagnósticos de SC se realizaron en niños menores de 7 días de vida (95,9%). Conclusión: La creciente incidencia de SG y SC en la Región de salud I, en el estado de Pará, apunta a la necesidad de mejoras en las acciones relacionadas con la prevención, diagnóstico, tratamiento y control de la enfermedad, así como en la atención prenatal.Objetivo: Descrever o perfil epidemiológico da sífilis em gestantes (SG) e congênita (SC) na Região de Saúde I, do estado Pará, Brasil, na série histórica de 2011 a 2020. Quanto à incidência da SC, houve crescimento significativo em Belém e Ananindeua (p<0,05), porém as variações nas taxas não foram significantes em Marituba (p=0,169), Benevides (p=0,632) e Santa Bárbara (p=0,731). Dentre as gestantes com sífilis, foi verificado um predomínio de mulheres de cor parda (78,5%); faixa etária de 20 a 29 anos (57,2%); grau de escolaridade intermediário (34,0%) e baixo (33,5%); com registro de “realização do pré-natal” (76,2%); que foram diagnosticadas no 3º trimestre da gestação (58%); e tiveram tratamento inadequado (48,8%). A maioria dos diagnósticos de SC foi realizado em menores de 7 dias de vida (95,9%).
