Influence of Atraumatic Restorative Treatment and its therapeutic indication against infected and affected tissue: a narrative review

Resumo: Atraumatic Restorative Treatment, known as ART (Atraumatic Restorative Treatment), is a technique that uses only manual excavators and curettes to remove decayed tissue. Subsequently, an adhesive restorative material is applied, such as glass ionomer cement (GIC), which has the advantage of releasing fluoride. This technique uses selective removal of carious tissue, removing only infected dentin and maintaining the affected dentin. Thus, the objective of this review is to carry out about ART, its characteristics, advantages and limitations during the therapeutic decision regarding the infected and affected tissue. A search was carried out through the PubMed and BVS databases, from 2013 to 2022, with the keywords: Dental Caries AND Dentin AND Caries-affected dentin. The ART technique is indicated for both primary and permanent teeth. It is a simple, easy-to-perform, low-cost, painless technique that does not require the use of a high-speed pen and the use of local anesthesia. This technique has some limitations such as the size and retention of the cavity, quality of the restorative material and, mainly, the low acceptance of the technique by the professional and community. Guidance should be monitored with control queries. It is possible to conclude that ART is a technique of wide social reach, reducing clinical time, being easy to apply both in the dental chair and in places without equipment. However, it requires training and qualification of the operator and assistant.

Posteriormente, se aplica un material adhesivo restaurador, como cemento de ionómero de vidrio (CIV), que tiene la ventaja de liberar flúor. Esta técnica utiliza la eliminación selectiva del tejido cariado, eliminando solo el tejido infectado y manteniendo el tejido afectado. Así, el objetivo de este trabajo es realizar una revisión bibliográfica sobre las ART, sus características, ventajas y limitaciones durante la decisión terapéutica respecto al tejido infectado y afectado. Se realizó una búsqueda a través de las bases de datos PubMed y BVS, de 2013 a 2022, con las palabras clave: Dental Caries AND Dentin AND Caries-affected dentin. La técnica ART está indicada tanto para dientes primarios como permanentes. Es una técnica sencilla, fácil de realizar, de bajo costo, indolora que no requiere el uso de un bolígrafo de alta velocidad y el uso de anestesia local. Esta técnica tiene algunas limitaciones como el tamaño y retención de la cavidad, la calidad del material restaurador y, principalmente, la baja aceptación de la técnica por parte del profesional y la comunidad. La orientación debe ser monitoreada con consultas de control. Es posible concluir que la TRA es una técnica de amplio alcance social, reduciendo el tiempo clínico, siendo de fácil aplicación tanto en el sillón odontológico como en lugares sin equipamiento. Sin embargo, requiere entrenamiento y calificación del operador y asistente.Tratamento Restaurador Atraumático, conhecido como ART (Atraumatic Restorative Treatment), trata-se de uma técnica que utiliza apenas escavadores e curetas manuais para realizar a remoção do tecido cariado. Posteriormente, é aplicado um material restaurador adesivo, como o cimento de ionômero de vidro (CIV) que apresenta como vantagem a liberação de flúor. Essa técnica utiliza a remoção seletiva de tecido cariado, removendo apenas tecido infectado e mantém o tecido afetado. Com isso, o objetivo desse trabalho é realizar uma revisão de literatura acerca do ART, suas características, vantagens e limitações durante a decisão terapêutica frente ao tecido infectado e afetado. Foi realizada uma pesquisa através das bases de dados PubMed e BVS, no período de 2013 a 2022, com as palavras chaves: Dental Caries AND Dentin AND Caries-affected dentin. A técnica ART é indicada tanto para dentes decíduos quanto para permanentes. É uma técnica simples, de fácil execução, de baixo custo, indolor, em que dispensa o uso da caneta de alta rotação e uso de anestesia local. Essa técnica apresenta algumas limitações como, o tamanho e retenção da cavidade, qualidade do material restaurador e, principalmente, a baixa aceitação da técnica por parte do profissional e comunidade. A orientação deve ser monitorada com consultas controle. É possível concluir que o ART é uma técnica de amplo alcance social, reduzindo tempo clínico, sendo de fácil aplicação tanto na cadeira odontológica como em locais sem equipamentos.
