Evaluation of the influence of torques and angulations in the insertions of the mini screws in bone cortical

Resumo: Anchorage has always been one of the limitations of orthodontics and in almost all orthodontic therapy, it is necessary to obtain it. With the discovery of osseous integration, a new anchoring model, the skeletal model, emerged. In an extra alveolar skeletal anchorage, the more oblique positioning of the mini-screws can increase the risk of deformation. The objective of this work was to analyze, in an in vitro study, the mechanical performance in the insertion and removal torques and to observe if the type of alloy and the insertion angles influence the use of these mini-screws in extra alveolar positions. 40 mini-screws of similar diameter and length (2.0x12mm), but of different alloys (20 of steel and 20 of titanium) were used, all inserted by a digital torque wrench in synthetic polyurethane bone cortex (40PCF). The insertions were made in two angles, one at 30º and the other from 60º to 30º. After obtaining the data, they were subjected to statistical analysis of variance, Generalized Estimation Equations, with a 95% confidence level and a 5% significance level. Statistical differences were observed in the analyzed factors. Among the factors analyzed, type of alloy and angulation of insertion, and with the data obtained we can conclude that regardless of the alloy, angulation is an essential factor in increasing the torques.

Con el descubrimiento de la integración ósea, surgió un nuevo modelo de anclaje, el modelo esquelético. En un anclaje esquelético alveolar adicional, la posición más oblicua de los mini-tornillos puede aumentar el riesgo de deformación. El objetivo de este trabajo fue analizar, en un estudio in vitro, el comportamiento mecánico en los pares de inserción y extracción y observar si el tipo de aleación y los ángulos de inserción influyen en el uso de estos minitornillos en posiciones extra alveolares. Se utilizaron 40 minitornillos de similar diámetro y longitud (2,0x12 mm), pero de diferentes aleaciones (20 de acero y 20 de titanio), todos insertados mediante llave dinamométrica digital en cortical ósea de poliuretano sintético (40PCF). Las inserciones se realizaron en dos ángulos, uno a 30º y otro de 60º a 30º. Luego de la obtención de los datos, se sometieron a análisis estadístico de varianza, Ecuaciones de Estimación Generalizada, con un nivel de confianza del 95% y un nivel de significancia del 5%. Se observaron diferencias estadísticas en los factores analizados. Entre los factores analizados, tipo de aleación y angulación de inserción, y con los datos obtenidos podemos concluir que independientemente de la aleación, la angulación es un factor fundamental para incrementar los torques.A ancoragem sempre foi uma das limitações da ortodontia e em quase toda terapia ortodôntica há necessidade de obtê-la. Com a descoberta da ósteo integração surgiu um novo modelo de ancoragem, a esquelética. Numa ancoragem esquelética extra alveolar o posicionamento mais oblíquo dos miniparafusos pode aumentar os riscos de deformação. O objetivo deste trabalho foi analisar, num estudo in vitro, o desempenho mecânico nos torques de inserção e remoção e observar se o tipo de liga e as angulações de inserção influenciam na utilização destes miniparafusos em posições extra alveolares. Foram utilizados 40 miniparafusos de diâmetro e comprimento semelhantes (2,0x12mm), porém de ligas diferentes (20 de aço e 20 de titânio), todos inseridos por um torquímetro digital em cortical óssea sintética de poliuretano (40PCF). As inserções foram feitas em duas angulações, uma a 30º e outra de 60º para 30º. Após obtenção dos dados, os mesmos foram submetidos à análise estatística de variância, Equações de Estimativas Generalizadas, com grau de confiança de 95% e nível de significância de 5%. Diferenças estatísticas foram observadas nos fatores analisados.
