New drug delivery systems, technological use and repositioning of chloroquine and hydroxychloroquine: An integrative review

Resumo: Chloroquine (CQ) and Hydroxychloroquine (HCQ) have been used historically to treat various conditions that differ from their original medical indication. In addition to their use as antimalarials, these drugs are used to treat autoimmune disorders and have recently gained attention due to in silico and in vitro evidence of their activity against SARS-CoV-2, a fact that has boosted their use rushedly in the treatment of COVID-19. The present study aimed to highlight the advances in research related to the repositioning of CQ and HCQ, their use as adjuvant therapy for treating other pathologies, and the development of new drug delivery systems. A search was performed in PubMed, Science Direct, and Web of Science using the keywords chloroquine, hydroxychloroquine, formulation and repositioning. Articles published in English from 2010 to 2020 that presented CQ or HCQ in the treatment of diseases or as components of formulations and distribution systems were included in the review. We obtained 788 articles, of which 69 were included in the study after screening and refining the search. According to the technological or clinical use of CQ and HCQ, these articles were categorized into four thematic groups: malaria, gene therapy, anticancer therapy, and immune-based diseases. In addition to studies on drug repositioning, we can highlight the articles with proposals for new formulations and drug delivery systems for conditions already treated with such molecules, with a particular focus on overcoming mechanisms of therapeutic resistance.

Además de su uso como antipalúdicos, estos medicamentos se utilizan para tratar trastornos autoinmunes y recientemente han llamado la atención debido a la evidencia in silico e in vitro de su actividad contra el SARS-CoV-2, hecho que ha impulsado su uso de forma apresurada en el tratamiento de COVID-19. El presente estudio tuvo como objetivo destacar los avances en la investigación relacionados con el reposicionamiento de CQ y HCQ, su uso como terapia adyuvante para el tratamiento de otras patologías y el desarrollo de nuevos sistemas de administración de fármacos. Se realizó una búsqueda en PubMed, Science Direct y Web of Science utilizando las palabras clave chloroquine, hydroxychloroquine, formulation y repositioning. Se incluyeron en la revisión artículos publicados en inglés de 2010 a 2020 que presentaban CQ o HCQ en el tratamiento de enfermedades o como componentes de formulaciones y sistemas de distribución. Se han obtenido 788 artículos, de los cuales 69 se incluyeron en el estudio después del refinamiento de la búsqueda y clasificación. Según el uso tecnológico o clínico de CQ y HCQ, estos artículos se categorizaron en cuatro grupos temáticos: malaria, terapia génica, terapia contra el cáncer y enfermedades de base inmunitaria. Además de los estudios sobre reposicionamiento de fármacos, podemos destacar los artículos con propuestas de nuevas formulaciones y sistemas de liberación de fármacos para patologías ya tratadas con este tipo de moléculas, con especial énfasis en la superación de los mecanismos de resistencia terapéutica.Cloroquina (CQ) e Hidroxicloroquina (HCQ) têm sido utilizadas historicamente para tratar diversas afecções que divergem da sua indicação médica original. Além do uso como antimaláricos, esses medicamentos são empregados na farmacoterapia de desordens autoimunes e recentemente ganharam atenção devido às evidências in silico e in vitro da atividade contra o SARS-CoV-2, fato que impulsionou sua utilização no tratamento da COVID-19 de forma precipitada. Sendo assim, o presente estudo teve como objetivo destacar os avanços das pesquisas relacionadas ao reposicionamento e uso como adjuvante da CQ e HCQ na terapia de outras patologias, assim como o desenvolvimento de novos sistemas de liberação para esses fármacos. Para isso, realizou-se uma busca nas bases de dados PubMed, Science Direct e Web of Science com os descritores chloroquine; hydroxychloroquine; formulation e repositioning. Foram incluídos na revisão artigos publicados na língua inglesa no período de 2010 a 2020 que apresentaram CQ e/ou HCQ no tratamento de doenças ou como componentes de formulações e sistema de distribuição. Obtiveram-se 788 artigos, dos quais 69 foram incluídos no estudo após refinamento da busca e triagem. Estes artigos foram categorizados, de acordo com o emprego tecnológico ou clínico da CQ e HCQ, em 4 grupos temáticos: malária; terapia gênica; terapia anticâncer e doenças de base imunológica.
