Difficulties in screening and diagnosis prostate cancer: an integrative review

Resumo: In Brazil, Prostate Cancer (PCa) is the second most common in men and has a high rate of morbidity and mortality. Therefore, the importance of early screening from the age of 50 is recommended. Given that, this study aims to analyze the screening and diagnosis difficulties encountered in Primary Health Care (PHC) for PCa prevention. This is an integrative literature review study, with a search performed mutually by two researchers in the Virtual Health Library (VHL), Scielo and PubMed, by crossing the descriptors “prostate cancer”, “screening” and “diagnosis”, using the Boolean operator and. To carry out the search and selection of articles, the following elegibility criteria were used: productions from the last two years; fully available for free; and published in Portuguese, English and Spanish. The initial search resulted in 488 articles, of which 105 met the adopted eligibility criteria. Of those, excluded the duplications, seven were selected to compose the corpus of the review because they meet the objective of the study. It was possible to identify factors that make it difficult for men to make a decision to perform PCa screening and diagnosis, especially beliefs, male self-vulnerability and medical communication. Besides, differences were found among health professionals about the standardization of criteria for screening. We emphasize the need for constant research in order to identify the difficulties of PCa screening and diagnosis.

Por ello, se recomienda la importancia del cribado precoz a partir de los 50 años. Así, este trabajo tiene como objetivo analizar las dificultades de cribado y diagnóstico encontradas en la Atención Primaria de Salud (APS) para la prevención del CaP. Se trata de un estudio integrador de revisión de la literatura, con búsqueda realizada mutuamente por dos investigadores en la Biblioteca Virtual en Salud (BVS), Scielo y PubMed, al cruzar los descriptores “cáncer de próstata”, “cribado” y “diagnóstico”, con el operador booleano y. Para realizar la búsqueda y selección de artículos, se utilizaron los siguientes criterios de elegibilidad: producciones de los últimos dos años; con texto completo disponible; y en portugués, inglés y español. La búsqueda inicial resultó en 488 artículos, de los cuales 105 cumplieron con los criterios de elegibilidad adoptados. De estos, excluyendo las duplicidades, siete fueron seleccionados para componer el corpus de la revisión porque cumplieron con el objetivo del estudio. Se logró identificar factores que dificultan que los hombres tomen la decisión de realizar el cribado y diagnóstico del CaP, en especial las creencias, la autovulnerabilidad masculina y la comunicación médica. Además, se encontraron diferencias entre los profesionales de la salud acerca de la estandarización de criterios para el cribado. Enfatizamos la necesidad de investigación constante para identificar las dificultades del cribado y diagnóstico del CaP.No Brasil, o Câncer de Próstata (CaP) é o segundo mais comum em homens e com alta taxa de morbimortalidade. Trata-se de um estudo de revisão integrativa da literatura, com busca realizada mutuamente por dois pesquisadores na Biblioteca Virtual em Saúde (BVS), Scielo e PubMed, por meio do cruzamento dos descritores “câncer de próstata”, “rastreamento” e “diagnóstico”, com o operador booleano and. Para a realização da busca e seleção dos artigos, foram utilizados os seguintes critérios de elegibilidade: produções dos últimos dois anos; com texto completo disponível; e nos idiomas português, inglês e espanhol. A busca inicial resultou em 488 artigos, dos quais 105 atenderam aos critérios de elegibilidade adotados. Destes, excluídas as duplicidades, sete foram selecionados para comporem o corpus da revisão por atenderem ao objetivo do estudo. Foi possível identificar fatores que dificultam a tomada de decisão dos homens para a realização do rastreio e diagnóstico do CaP, destacando-se crenças, autovulnerabilidade do sexo masculino e a comunicação médica. Além disso, constataram-se divergências entre profissionais de saúde sobre a padronização de critérios para o rastreio.
