Fournier's gangrene: New therapeutic approaches with the use of negative pressure and hyperbaric oxygen therapy

Resumo: Objective: To review the available scientific literature, in order to synthesize some forms of adjuvant treatment available in the management of patients with FG. Methods: research on some indexing platforms, a total of 17 articles, published in the last 7 years, which related to adjuvant methods to the treatment of Fournier's Gangrene, in particular hyperbaric oxygen therapy and wound treatment therapy with the use, were selected of negative pressure. Results and Discussion: The methods used as adjuvants for wound closure contribute positively to the recovery of patients with FG, when reported after indication. The wound closure therapy assisted in the reduction, if useful in controlling the environment of recovery from the injury and reducing the time of clinical treatment without interfering in the mortality of the disease. Hyperbaric oxygen therapy has a direct antibacterial effect on anaerobics, improves the phagocytic action of neutrophils, increases angiogenesis and reduces edema. Negative pressure therapy is used in the acute and repair phases, accelerating the appearance of granulation tissue. Conclusion: The use of adjuvant methods to the treatment of Fournier's Gangrene is promising and, with exceptions, demonstrate better wound recovery when compared to patients who do not use them, and after professional and individualized clinical evaluation of the cases, such methods can be allies in the recovery of the sick.

Métodos: Después de investigar algunas plataformas de indexación, se seleccionaron un total de 17 artículos, publicados en los últimos 7 años, que estaban relacionados con métodos coadyuvantes al tratamiento de la gangrena de Fournier, en particular la oxigenoterapia hiperbárica y la terapia de tratamiento de heridas con uso de presión negativa. Resultados y Discusión: Los métodos utilizados como adyuvantes para el cierre de heridas contribuyen positivamente a la recuperación de los pacientes con GF, cuando se aplican después de una adecuada indicación. La terapia de cierre de heridas asistida por vacío demostró ser útil para controlar el entorno de recuperación de la herida y reducir el tiempo de tratamiento clínico sin interferir con la mortalidad por enfermedad. La oxigenoterapia hiperbárica tiene un efecto antibacteriano directo sobre los anaerobios, mejora la acción fagocítica de los neutrófilos, aumenta la angiogénesis y reduce el edema. La terapia de presión negativa se utiliza en la fase aguda y de reparación, acelerando la aparición del tejido de granulación. Conclusión: El uso de métodos adyuvantes para el tratamiento de la gangrena de Fournier es prometedor y, salvo excepciones, demuestra una mejor recuperación de la herida en comparación con los pacientes que no los utilizan, y tras una evaluación clínica profesional e individualizada de los casos, dichos métodos pueden ser aliados en la recuperación de los enfermos.Objetivo: Revisar a literatura científica disponível, com intuito de sintetizar algumas formas de tratamento adjuvantes disponíveis na abordagem do paciente com GF. Métodos: Após pesquisas em algumas plataformas indexadoras, foram selecionados, ao todo 17 artigos, publicados nos últimos 7 anos, que se relacionaram com métodos adjuvantes ao tratamento da Gangrena de Fournier, em especial a oxigenoterapia hiperbárica e a terapia de tratamento de feridas com o uso de pressão negativa. Resultados e Discussão: Os métodos utilizados como coadjuvantes para o fechamento das feridas contribuem positivamente para recuperação dos pacientes com GF, quando aplicados após indicação adequada. A terapia de fechamento de feridas assistido à vácuo, se mostrou útil em controlar o ambiente de recuperação da lesão e reduzir o tempo de tratamento clínico sem interferir na mortalidade da doença. A oxigenoterapia hiperbárica exerce efeito antibacteriano direto sobre os anaeróbicos, melhora a ação fagocítica dos neutrófilos, aumenta a angiogênese e reduz o edema. A terapia de pressão negativa é utilizada na fase aguda e de reparo, acelerando o surgimento de tecido de granulação.
