Use of alternative device for maintenance of nasal permeability in previous nasal packing: case report

Resumo: | The treatment of nasal fractures involves bone repositioning and its stabilization, performed through nasal packing. However, this can be extremely uncomfortable as it obliterates the nostrils and reduces or impedes nasal breathing. Devices that ensure the maintenance of airway permeability bring greater comfort to the patient in the postoperative period. Therefore, the objective of this study is to describe the use of an alternative device to preserve nasal ventilation in patients with nasal fractures, as well as to demonstrate a clinical case. The methodology used in this work was carried out in a qualitative and descriptive way, through direct collection of patient data. A 62-year-old male patient, victim of a fall from standing height, was diagnosed with a fracture of the proper bones of the nose associated with a deviated septum. He was submitted to general anesthesia, closed bone reduction and stabilization with anterior nasal packing by placing a nasogastric tube in the nostrils directed to the oropharynx region and sterile gauze directed to the nasal bones. The patient evolved well, with improved aesthetic and functional complaints. It can be concluded that the alternative device used in this clinical case promoted greater patient comfort, guaranteed the maintenance of nasal patency, in addition to its low cost and easy availability. |

Sin embargo, esto puede ser extremadamente incómodo ya que oblitera las fosas nasales y reduce o dificulta la respiración nasal. Los dispositivos que aseguran el mantenimiento de la permeabilidad de la vía aérea aportan mayor comodidad al paciente en el postoperatorio. Por tanto, el objetivo de este estudio es describir el uso de un dispositivo alternativo para preservar la ventilación nasal en pacientes con fracturas nasales, así como demostrar un caso clínico. La metodología utilizada en este trabajo se llevó a cabo de forma cualitativa y descriptiva, mediante la recogida directa de datos de pacientes. Un paciente masculino de 62 años, víctima de una caída desde una altura de pie, fue diagnosticado con una fractura de los huesos propios de la nariz asociada a un tabique desviado. Se le sometió a anestesia general, reducción ósea cerrada y estabilización con taponamiento nasal anterior mediante la colocación de sonda nasogástrica en las fosas nasales dirigida a la región orofaríngea y gasa estéril dirigida a los huesos nasales. El paciente evolucionó bien, con mejoría de las molestias estéticas y funcionales. Se puede concluir que el dispositivo alternativo utilizado en este caso clínico promovió una mayor comodidad del paciente, garantizó el mantenimiento de la permeabilidad nasal, además de su bajo costo y fácil disponibilidad.O tratamento das fraturas nasais envolve o reposicionamento ósseo e sua estabilização, realizada através do tamponamento nasal. Entretanto, esse pode ser extremamente desconfortável por obliterar as narinas e reduzir ou impedir respiração nasal. Dispositivos que garantem a manutenção da permeabilidade das vias aéreas trazem maior conforto ao paciente no pós-operatório. Logo, o objetivo deste trabalho é descrever o uso de dispositivo alternativo para preservação da ventilação nasal de pacientes com fratura nasal, bem como demonstrar um caso clínico. A metodologia utilizada neste trabalho foi realizada de forma qualitativa e descritiva, através da coleta direta dos dados do paciente. Paciente do sexo masculino, 62 anos, vítima de queda da própria altura, foi diagnosticado com fratura dos ossos próprios do nariz associada a desvio de septo. O mesmo foi submetido a anestesia geral, redução óssea fechada e estabilização com tamponamento nasal anterior através da colocação de sonda nasogástrica nas narinas direcionada para região de orofaringe e gaze estéril direcionada aos ossos nasais. O paciente evoluiu bem, com melhora da queixa estética e funcional.
