Development of a low-cost customized orthosis for gait facilitation in patients with hip adductor spasticity

Resumo: | The main purpose of the present work was to describe a protocol for making a prototype of a low-cost customized orthosis to facilitate gait in patients with spastic paraplegia in the lower limbs, leading the reader to understand the indication for use, the making, and the components of the orthosis, which has the objective of reducing some of the specific difficulties of the lower limbs in walking and providing low costs in its manufacture. This is a study that was developed following a quantitative approach, parallel to a literature search on aspects relevant to orthosis manufacturing. The study was guided through the prototyping stages: literature review on the theme, product manufacturing, and technical adjustments before applicability. This study presented the description of a low cost method for the development of a prototype of a customized orthosis for lower limbs. After the first prototype was ready, the first fitting of the orthosis was performed on the authors, and, after the observation of the need for adjustments, a second prototype and a third prototype were made. The first prototype was created with a piece of mattress foam tied to an elastic, which was improved, and from it the second one was created with Velcro on the ends of the elastic, and then improved with a cover with jeans fabric and elastic bands with a nylon fabric suspender attached to a fabric belt, which prevents excessive abduction of the hips during gait. It is evident that this resource still needs further studies and patenting process so that its application can be guaranteed with full scientific support, but in general, it was possible to develop a customized orthosis with low-cost materials and tools, where the described procedures may help in the development of assistive products and provide dissemination of this knowledge in institutions that dispense orthoses. |

Se trata de un estudio que se desarrolló siguiendo un enfoque cuantitativo paralelo a una investigación literaria sobre aspectos relevantes para la fabricación de ortesis. El estudio fue guiado a través de las etapas de creación de prototipos: revisión de la literatura sobre el tema, fabricación del producto y ajustes técnicos previos a la aplicabilidad. Este estudio presenta la descripción de un método de bajo coste para el desarrollo de un prototipo de ortesis personalizada para miembros inferiores. Una vez que el primer prototipo estuvo listo, se realizó la primera adaptación de la órtesis a los autores y, tras la observación de la necesidad de ajustes, se realizaron un segundo prototipo y un tercer prototipo. El primer prototipo se creó con un trozo de espuma de colchón atado a una banda elástica, que se mejoró, y a partir del cual se creó el segundo con velcro en los extremos de la banda elástica, y luego se mejoró con una funda con tela de jeans y bandas elásticas con un tirante de tela de nylon fijado en un cinturón de tela, que evita la abducción excesiva de las caderas durante la marcha. Es evidente que este recurso aún necesita de nuevos estudios y de un proceso de patentamiento para que su aplicación pueda ser garantizada con pleno respaldo científico, pero en general, fue posible desarrollar una órtesis personalizada con materiales y herramientas de bajo costo, donde los procedimientos descritos pueden ayudar en el desarrollo de productos de asistencia y proporcionar la difusión de este conocimiento en las instituciones que dispensan órtesis.O presente trabalho teve a finalidade principal de descrever um protocolo da confecção de um protótipo de órtese personalizada de baixo custo para facilitação da marcha em pacientes com paraplegia espástica em membros inferiores, levando o leitor a compreender a indicação de uso, a confecção e os componentes da órtese, a qual tem o objetivo de diminuir algumas das dificuldades de membros inferiores específicas na deambulação e proporcionar baixos custos em suas fabricações. Trata-se de um estudo que foi desenvolvido seguindo um enfoque quantitativo paralelo a uma pesquisa literária sobre aspectos relevantes à confecção de órteses. O estudo guiou-se através das etapas de prototipagem de: revisão da literatura sobre a temática, confecção do produto e ajustes técnicos pré aplicabilidade. Este estudou apresentou a descrição de um método de baixo custo para o desenvolvimento de um protótipo de órtese personalizada para membros inferiores. Após o primeiro protótipo estar pronto, foi realizada a primeira prova da órtese nos autores, e, após a observação da necessidade de ajustes, foi confeccionado um segundo protótipo e um terceiro protótipo. Foi criado o primeiro protótipo com um pedaço de espuma de colchão amarrada a um elástico, que foi aperfeiçoado e a partir dele criado o segundo com velcro nas pontas do elástico e em seguida aperfeiçoado com uma capa com tecido jeans e elásticos com um suspensório de tecido em nylon fixado em um cinto de tecido, que impede abduções excessivas dos quadris durante a marcha.
