The pathophysiological integration between COVID-19 and Type 2 Diabetes Mellitus and the influence on patient prognosis: A literature review

Resumo: Introduction: The pandemic of COVID-19 shook the foundations of modern societies, bringing numerous questions about its functioning, which should be analyzed, especially by the interrelation with chronic diseases such as Diabetes. Objective: This study aimed to evaluate the pathogenic convergence points between SARS-CoV-2 infection and Type 2 Diabetes Mellitus, as well as the effect on patient prognosis. Materials and Methods: This is an integrative literature review about the pathophysiological integration between COVID-19 and Type 2 Diabetes Mellitus. The PICO strategy was used to develop the guiding question. Moreover, the descriptors "COVID-19"; "Diabetes Mellitus"; "Physiology" were cross-referenced with the National Library of Medicine (PubMed MEDLINE), Scientific Electronic Library Online (SCIELO), Ebscohost, Google Scholar, and Virtual Health Library (VHL) databases. Results and Discussion: A significant portion of the articles indicated an interrelationship between the pathophysiological mechanisms of COVID-19 and Type 2 Diabetes Mellitus, especially in the exacerbation of the immune response and direct damage to pancreatic cells, which result in a worse prognosis of the viral infection. Conclusion: It was possible to notice that both diseases have pathogenic interaction points, reinforcing the need to re-signify the health-disease process, aiming to understand the proper functioning of the pathophysiological mechanisms of each one of them.

Objetivo: El presente estudio se propuso evaluar los puntos de convergencia patogénica entre la infección por SARS-CoV-2 y la diabetes mellitus de tipo 2, así como el efecto sobre el pronóstico de los pacientes. Materiales y Métodos: Se trata de una revisión bibliográfica integradora sobre la integración fisiopatológica entre la COVID-19 y la Diabetes Mellitus Tipo 2. Se utilizó la estrategia PICO para desarrollar la pregunta guía. Además, se realizó el cruce de los descriptores "COVID-19"; "Diabetes Mellitus"; "Fisiologia", en las bases de datos National Library of Medicine (PubMed MEDLINE), Scientific Eletronic Library Online (SCIELO), Ebscohost, Google Scholar y Biblioteca Virtual de Salud (BVS). Resultados y Discusión: Una parte significativa de los artículos indicaron una interrelación entre los mecanismos fisiopatológicos de la COVID-19 y la Diabetes Mellitus Tipo 2, especialmente en la exacerbación de la respuesta inmune y el daño directo a las células pancreáticas, que resultan en un peor pronóstico de la infección viral. Conclusión: Se pudo observar que las dos enfermedades tienen puntos de interacción patogénica, reforzando la necesidad de resignificar el proceso salud-enfermedad, buscando comprender el correcto funcionamiento de los mecanismos fisiopatológicos de cada una de ellas.Introdução: A pandemia da COVID-19 abalou os alicerces das sociedades modernas, trazendo inúmeros questionamentos acerca do seu funcionamento, os quais devem ser analisados, sobretudo pela interrelação com doenças crônicas, como o Diabetes. Objetivo: O presente estudo teve como objetivo avaliar os pontos de convergência patogênica entre a infecção pelo SARS-CoV-2 e o Diabetes Mellitus Tipo 2, bem como o efeito sobre o prognóstico dos pacientes. Materiais e Métodos: Trata-se de uma revisão integrativa de literatura acerca da integração fisiopatológica entre a COVID-19 e o Diabetes Mellitus Tipo 2. Utilizou-se a estratégia PICO para a elaboração da pergunta norteadora. Ademais, realizou-se o cruzamento dos descritores “COVID-19”; “Diabetes Mellitus”; “Fisiologia”, nas bases de dados National Library of Medicine (PubMed MEDLINE), Scientific Eletronic Library Online (SCIELO), Ebscohost, Google Scholar e Biblioteca Virtual de Saúde (BVS). Resultados e Discussão: Uma parcela significativa dos artigos indicou uma interrelação entre os mecanismos fisiopatológicos da COVID-19 e o Diabetes Mellitus Tipo 2, sobretudo na exacerbação da resposta imune e dos danos diretos às células pancreáticas, os quais resultam em pior prognóstico da infecção viral.
