Challenges of the multidisciplinary team in palliative care in Brazil: integrative review

Resumo: Objective: To identify, in the literature, the challenges faced by health professionals when working in a multidisciplinary team of palliative care. Methods: This is an integrative review, carried out through searches in the Latin American and Caribbean Literature on Health Sciences, Medical Literature Analysis and Retrieval System Online and Scientific Electronic Library Online databases, during the month of November, 2021. Results: After the searches, 78 studies were found. Of these, 20 were included because they met the purpose of the study and proposed criteria. Such research analyzed different professional categories, with emphasis on nurses and doctors, followed by physiotherapists, nutritionists, nursing technicians, psychologists, occupational therapists and pharmacists. Among the challenges investigated, there is a lack of professional training and communication problems within the multidisciplinary team. In addition, the most pointed challenge was the lack of standardization of protocols for indicating and conducting palliative care, as well as the need for a better structure for reception and communication with family members. Conclusion: Therefore, the results found demonstrate that the professionals of the multidisciplinary team in palliative care face challenges related to aspects such as: professional training/qualification and operationalization of the teamwork routine; the field of action and the deficient support network for the patient; and communication/reception to patients and family members. This evidence demonstrates the need to improve professional qualification in palliative care. Thus, the dissemination of knowledge about palliative care can contribute to preventing or mitigating the challenges that may arise during professional practice.

Métodos: Esta es una revisión integradora, realizada a través de búsquedas en las bases de datos Literatura Latinoamericana y del Caribe en Ciencias de la Salud, Medical Literature Analysis and Retrieval System Online y Scientific Electronic Library Online, durante el mes de noviembre de 2021. Resultados: Después de las búsquedas , se encontraron 78 estudios. De estos, se incluyeron 20 porque cumplieron con el propósito del estudio y los criterios propuestos. Dicha investigación analizó diferentes categorías profesionales, con énfasis en enfermeros y médicos, seguidos de fisioterapeutas, nutricionistas, técnicos de enfermería, psicólogos, terapeutas ocupacionales y farmacéuticos. Entre los desafíos investigados, se encuentran la falta de formación profesional y problemas de comunicación dentro del equipo multidisciplinario. Además, el desafío más señalado fue la falta de estandarización de los protocolos para indicar y realizar los cuidados paliativos, así como la necesidad de una mejor estructura de recepción y comunicación con los familiares. Conclusión: Por lo tanto, los resultados encontrados demuestran que los profesionales del equipo multidisciplinario en cuidados paliativos enfrentan desafíos relacionados con aspectos tales como: formación/calificación profesional y operacionalización de la rutina de trabajo en equipo; el campo de acción y la deficiente red de apoyo al paciente; y comunicación/recepción a pacientes y familiares. Esta evidencia demuestra la necesidad de mejorar la calificación profesional en cuidados paliativos. Así, la difusión del conocimiento sobre los cuidados paliativos puede contribuir a prevenir o mitigar los desafíos que puedan surgir durante la práctica profesional.Objetivo: Identificar, na literatura, os desafios enfrentados pelos profissionais de saúde quanto à atuação numa equipe multiprofissional de cuidados paliativos. Resultados: Após as buscas, foram encontrados 78 estudos. Destes, 20 foram incluídos por atenderem ao objetivo do estudo e critérios propostos. Tais pesquisas analisaram diferentes categorias profissionais, com destaque para enfermeiros e médicos, seguidos pelos fisioterapeutas, nutricionistas, técnicos de enfermagem, psicólogos, terapeutas ocupacionais e farmacêuticos. Entre os desafios averiguados, observa-se a falta de capacitação profissional e problemas de comunicação dentro da equipe multiprofissional. Ademais, o desafio mais apontado foi a ausência de uniformização de protocolos para indicação e condução dos cuidados paliativos, bem como a necessidade de uma melhor estrutura para o acolhimento e comunicação com os familiares. Conclusão: Portanto, os resultados encontrados demonstram que os profissionais da equipe multiprofissional em cuidados paliativos enfrentam desafios relacionados à aspectos como: formação/qualificação profissional e operacionalização da rotina de trabalho em equipe; ao campo de atuação e a rede deficitária de suporte ao paciente; e de comunicação/acolhimento aos pacientes e familiares. Estas evidências demonstram a necessidade de melhorar a qualificação profissional em cuidados paliativos.
