Pre-surgical physical therapy treatment in adult with late diagnosis of bilateral Bochdalek's hernia: a case study

Resumo: Bochdalek's Hernia (BH) is characterized by a defect in the posterolateral diaphragmatic foramen leading to evisceration of the abdominal contents into the thoracic cavity. The treatment is provided, according to the extent of the hernia and the degree of involvement of the structures, through a simple surgery by videolaparoscopy or, in more severe cases, the use of polypropylene meshes for the repair. The aim of the present study was to report the late presentation of a case of bilateral BH in an adult and the benefits of chest physiotherapy in the pre-surgical context. It can be concluded that cardiorespiratory reeducation is beneficial in the pre-surgical approach to BH, providing benefits such as improved functional capacity, increased chest expansion, improved quality of life and consequent reduction of surgical risk factors.

El tratamiento se da, según la extensión de la hernia y el grado de afectación de las estructuras, mediante una simple cirugía por videolaparoscopia o en casos más severos el uso de mallas de polipropileno para la reparación. El objetivo del presente estudio fue reportar la presentación tardía de un caso de HB bilateral en un adulto y los beneficios de la fisioterapia torácica en el contexto prequirúrgico. Se puede concluir que la reeducación cardiorrespiratoria es beneficiosa en el abordaje prequirúrgico de la HB, aportando beneficios como mejora de la capacidad funcional, aumento de la expansión torácica, mejora de la calidad de vida y consecuente reducción de los factores de riesgo quirúrgico.A Hérnia de Bochdalek (HB) é caracterizada por um defeito no forame diafragmático posterolateral levando a evisceração do conteúdo abdominal para a cavidade torácica. O tratamento se dá, de acordo com a extensão da hérnia e o grau de comprometimento das estruturas, por meio de uma cirúrgica simples por videolaparoscopia ou em casos mais severos o uso de telas de polipropileno para o reparo. O objetivo do presente estudo, foi relatar a apresentação tardia de um caso de HB bilateral em adulto e os benefícios da fisioterapia respiratória em âmbito pré-cirúrgico.
