Global Burden of Disease: Estimate and time trend of the burden of leprosy in the state of Alagoas, Brazil from 2001 to 2019

Resumo: The leprosy burden continues to grow in Brazil, especially in endemic areas, causing a large number of patients with physical disabilities. Thus, we aim to measure and characterize the burden related to leprosy and analyze its temporal patterns in Alagoas, Brazil, between 2001 and 2019. For this, we measure years of life lost (YLL), years lived with disability (YLD) and years of Disability-Adjusted Life (DALY) for leprosy in Alagoas from 2001 to 2019. The metrics were also presented as rates per 100,000 population. In addition, we use the segmented log-linear regression model to assess temporal trends. We estimate for the state about 968.48 YLLs, 88.02 YLDs, and 1056.49 DALYs for leprosy in grade 1 and 2 disability. The highest rates per 100,000 inhabitants of the leprosy burden were in males. However, women showed a very high relative percentage increase in DALY between 2001 and 2019. As the age group increases, there was also an increase in YLD rates for grade 1 and 2 disability. In addition, the time trend of YLDs in the general population was increasing for disability grade 1 and 2 and grade 1. Our data highlight the still existing impact of leprosy and may contribute to reducing the burden and improving disease control in Brazil.

De esta manera, buscamos medir y caracterizar la carga relacionada con la lepra y analizar sus patrones temporales en Alagoas, Brasil, entre 2001 y 2019. Para ello medimos años de vida perdidos (YLL), años vividos con discapacidad (YLD) y años de vida ajustada por discapacidad (DALY) para la lepra en Alagoas de 2001 a 2019. Las métricas también se presentaron como tasas por 100.000 habitantes. Además, utilizamos el modelo de regresión logarítmica lineal segmentado para evaluar las tendencias temporales. Estimamos para el estado alrededor de 968,48 YLLs, 88,02 YLDs y 1056,49 DALYs para la lepra en discapacidades de grado 1 y 2. Las tasas más altas por cada 100.000 habitantes de la carga de lepra se registraron en los hombres. Sin embargo, las mujeres mostraron un aumento porcentual relativo muy alto en DALY entre 2001 y 2019. A medida que aumenta el grupo de edad, también hubo un aumento en las tasas de YLD para discapacidades de grado 1 y 2. Además, la tendencia temporal de los YLDs en la población general estaba aumentando para los grados de discapacidad 1 y 2 y el grado 1. Nuestros datos destacan el impacto aún existente de la lepra y pueden contribuir a reducir la carga y mejorar el control de la enfermedad en Brasil.A carga da hanseníase continua a crescer no Brasil, principalmente em áreas endêmicas, ocasionando um grande número de pacientes com incapacidades físicas. Dessa forma, objetivamos mensurar e caracterizar a carga relacionada à hanseníase e analisar seus padrões temporais em Alagoas, Brasil, entre 2001 a 2019. Para isso nós mensuramos os anos de vida perdidos (YLL), anos vividos com incapacidade (YLD) e anos de vida ajustados por incapacidade (DALY) para hanseníase em Alagoas entre 2001 a 2019. As métricas também foram apresentadas como taxas por 100.000 habitantes. Além disso, nós usamos o modelo de regressão log-linear segmentado para avaliar as tendências temporais. Nós estimamos para o estado cerca de 968,48 YLLs, 88,02 YLDs e 1056,49 DALYs por hanseníase em grau 1 e 2 de incapacidade. As maiores taxas por 100 mil habitantes da carga da hanseníase foram no sexo masculino. No entanto, as mulheres apresentaram aumento percentual relativo de DALY altíssimo entre 2001 e 2019. Conforme aumento da faixa etária, também ocorreu aumento das taxas de YLD para o grau 1 e 2 de incapacidade. Além disso, a tendência temporal dos YLDs na população geral foi crescente para o grau 1 e 2 de incapacidade e grau 1.
