Renal cystadenocarcinoma-nodular dermatofibrosis syndrome in a German Shepherd canine - Case report

Resumo: The objective of this study is to describe the clinical, ultrasound, serum and anatomopathological aspects of renal cystadenocarcinoma-nodular dermatofibrosis (SCRDN) in an eight-year-old female German Shepherd dog treated at the Hospital Veterinário of Universidade de Cruz Alta (HV/ UNICRUZ). The animal had a history of abdominal pain and vomiting. Clinical examination revealed a nodule in the left hind limb. The animal was submitted to complementary examinations of ultrasound, hematology, biochemical profile and fine needle aspiration cytology (FNAC) of the cutaneous nodule. Ultrasonography detected enlargement of the kidneys and the presence of anechoic vacuoles distributed throughout the organ. In the biochemical profile there was an increase in the levels of urea and creatinine. Due to the evolution of the condition, the patient was euthanized and referred to the Laboratório de Patologia Veterinária at UNICRUZ (LPV/UNICRUZ). During necropsy, enlarged kidneys were observed, with a bossed surface and areas of omentum adherence. When cut, they were cystic, multiloculated and filled with material either liquid and red or hemorrhagic, with a necrotic appearance. Fragments of all organs were collected, fixed in 10% buffered formalin. After 48 hours, the samples were routinely cleaved and processed to make histological slides. In the microscopic evaluation of the kidney, areas of cystification were observed, covered by neoplastic cuboidal epithelium and, occasionally, solid areas of these tumor cells in the middle of the renal parenchyma. We also observed areas of interstitial fibrosis, tubular degeneration and necrosis and thickening of the renal capsule, with areas of subcapsular hemorrhage. The diagnosis of renal cystadenocarcinoma was based on the history, imaging and biochemical tests, confirmed by macroscopic and histopathological findings.

El animal tenía antecedentes de dolor abdominal y vómitos. El examen clínico reveló un nódulo en la extremidad posterior izquierda. El animal fue sometido a exámenes complementarios de ultrasonido, hematología, perfil bioquímico y citología por aspiración con aguja fina (CAAF) del nódulo cutáneo. La ecografía detectó agrandamiento de los riñones y la presencia de vacuolas anecoicas distribuidas por todo el órgano. En el perfil bioquímico hubo aumento de urea y creatinina. Debido a la evolución del cuadro, el paciente fue sacrificado y remitido al Laboratório de Patologia Veterinária de la UNICRUZ (LPV/UNICRUZ). Durante la necropsia se observaron riñones aumentados de tamaño, con superficie abombada y áreas de adherencia del epiplón. Al corte eran quísticos, multiloculados y llenos de material líquido y rojo o hemorrágico, de aspecto necrótico. Se recogieron fragmentos de todos los órganos, se fijaron en formalina tamponada al 10%. Después de 48 horas, las muestras se escindieron y procesaron de forma rutinaria para hacer portaobjetos histológicos. En la evaluación microscópica del riñón se observaron áreas de cistificación recubiertas por epitelio cúbico neoplásico y, ocasionalmente, áreas sólidas de estas células tumorales en medio del parénquima renal. También se observaron áreas de fibrosis intersticial, degeneración y necrosis tubular y engrosamiento de la cápsula renal, con áreas de hemorragia subcapsular. El diagnóstico de cistadenocarcinoma renal se basó en la anamnesis, pruebas de imagen y bioquímicas, confirmadas por hallazgos macroscópicos e histopatológicos.O objetivo deste trabalho é descrever os aspectos clínicos, ultrassonográficos, séricos e anatomopatológicos de cistadenocarcinoma renal-dermatofibrose nodular (SCRDN) em um canino fêmea, da raça Pastor Alemão de oito anos de idade atendido no Hospital Veterinário da Universidade de Cruz Alta (HV/UNICRUZ). O animal apresentava histórico de algia abdominal e vômito. No exame clínico observou-se um nódulo no membro posterior esquerdo. O animal foi submetido a exames complementares de ultrassonografia, hematologia, perfil bioquímico e citologia aspirativa por agulha fina (CAAF) do nódulo cutâneo. Na ultrassonografia foi detectado aumento dos rins e presença de vacúolos anecoicos distribuídos pelo órgão. No perfil bioquímico havia aumento nos níveis de ureia e creatinina. Devido evolução do quadro a paciente foi eutanasiada e encaminhada ao Laboratório de Patologia Veterinária da UNICRUZ (LPV/UNICRUZ). Durante a necropsia observou-se rins aumentados, com superfície bosselada e áreas de aderência do omento. Ao corte estes eram císticos, multiloculados e preenchidos por material ora líquido e vinhoso ora hemorrágico, de aspecto necrótico. Fragmentos de todos os órgãos foram coletados, fixados em formalina 10% tamponada. Após 48 horas as amostras foram clivadas e processadas rotineiramente para confecção de lâminas histológicas. Na avaliação microscópica do rim observou-se áreas de cistificação, revestidas por epitélio cuboidal neoplásico e, ocasionalmente, áreas sólidas destas células tumorais em meio ao parênquima renal. Observou-se, ainda, áreas de fibrose intersticial, degeneração e necrose tubular e espessamento da cápsula renal, com áreas de hemorragia subcapsular.
