Aesthetic transformation of the smile with strategic additions in composite resin: Clinical case

Resumo: Diastemas are interdental spaces larger than 0.5mm with a multifactorial etiology that can compromise the aesthetics of the smile. Various forms of treatment have been reported in the literature for their correction, such as: orthodontics, surgery, use of ceramic laminates or composite resins. However, the therapeutic choice depends on the identification of its etiology. The aim of this study is to report a clinical case of closing diastemas with direct composite resin, using a silicone palatal guide, after orthodontic treatment. Initially, it was carried out: color selection of the composite resin with the hydrated dental surface; absolute isolation of the operative field; prophylaxis with pumice paste and water; conditioning with 37% phosphoric acid (30 seconds in enamel); removal with water / air spray for twice the conditioning time; drying; application of universal adhesive (two layers); light curing (20 seconds); making the palatal shell, shaping the contours and surfaces in composite resin; removal of isolation from the operative field; occlusal adjustment and polishing. It is concluded that: 1) The use of composite resins for the closure of diastemas and dental remodeling with incisal enlargement represents. an affordable choice and can be an alternative for orthodontic finishes, in closing diastemas. 2) The use of the silicone guide makes it easier to define height; contours and construction of the palatal wall, minimizing the need for adjustments. 3) The therapy employed is effective for the aesthetic improvement of the smile, being a conservative, reversible, predictable technique that has led to patient satisfaction.

Se han reportado en la literatura diversas formas de tratamiento para su corrección, tales como: ortodoncia, cirugía, uso de laminados cerámicos o resinas compuestas. Sin embargo, la elección terapéutica depende de la identificación de su etiología. El objetivo de este estudio es reportar un caso clínico de cierre de diastemas con resina compuesta directa, utilizando una guía palatina de silicona, luego de un tratamiento de ortodoncia. Secuencialmente, se realizó: selección de color de la resina compuesta con la superficie dental hidratada; absoluto aislamiento del campo operatorio; profilaxis con pasta pómez y agua; acondicionamiento con ácido fosfórico al 37% (30 segundos en esmalte); eliminación con agua / aire pulverizado durante el doble del tiempo de acondicionamiento; el secado; aplicación de adhesivo universal (dos capas); fotopolimerización (20 segundos); hacer la concha palatina, dar forma a los contornos y superficies en resina compuesta; eliminación del aislamiento del campo operatorio; ajuste oclusal y pulido. Se concluye que: 1) El uso de resinas compuestas para el cierre de diastemas y remodelación dental con agrandamiento incisal representa. una opción asequible y puede ser una alternativa para los acabados de ortodoncia, en el cierre de diastemas. 2) El uso de la guía de silicona facilita la definición de la altura; contornos y construcción de la pared palatina, minimizando la necesidad de ajustes. 3) La terapia empleada es eficaz para la mejora estética de la sonrisa, siendo una técnica conservadora, reversible, predecible que ha llevado a la satisfacción del paciente.Diastemas são espaços interdentais maiores que 0,5mm com etiologia multifatorial que podem comprometer a estética do sorriso. Várias formas de tratamento têm sido relatadas na literatura para sua correção, como: ortodontia, cirurgia, uso de laminados cerâmicos ou resinas compostas. Entretanto, a escolha terapêutica está na dependência da identificação de sua etiologia. O objetivo deste estudo é relatar um caso clínico de fechamento de diastemas com resina composta direta, utilizando guia palatina em silicone, pós-tratamento ortodôntico. Inicialmente foi realizado: seleção de cor da resina composta com a superfície dental hidratada; isolamento absoluto do campo operatório; profilaxia com pasta de pedra pomes e água; condicionamento com ácido fosfórico 37% (30 segundos em esmalte); remoção com spray de água/ar pelo dobro do tempo de condicionamento; secagem; aplicação do adesivo universal (duas camadas); fotopolimerização (20 segundos); confecção da concha palatina, modelação do contornos e superfícies em resina composta; remoção do isolamento do campo operatório; ajuste oclusal e polimento.
