The main neurological manifestations resulting from Cerebral Malaria in Brazil: an integrative review

Resumo: Objective: To understand the main neurological manifestations of cerebral malaria in Brazil. Methodology: This is an integrative review that searched literature between the years 2007 to 2022, through the descriptors “malaria”, “diseases of the nervous system” and “Brazil”, researched on LILACS, PUBMED and SciELO platforms. Publications with full text available, published between 2007 and 2022, in English, Portuguese and Spanish were used. In addition, studies that did not address the research proposal, as well as repeated or duplicated studies were excluded. Results: The neurological manifestations most frequently reported in the studies presented are characterized by cerebral edema, seizures, coma, cognitive deficit, hippocampal impairment and disturbance of consciousness. Furthermore, the clinical signs and symptoms of severity differ in adults and children, and in both cases they can configure the lethal form of the pathology. Conclusion: The main neurological manifestations of the disease involve coma, seizures, changes in muscle tone, loss of reflexes, and unspecified behavioral and motor dysfunctions. Despite the neurological clinical signs and symptoms found, cerebral malaria is still a neglected disease, with differences in prognosis between child and adult age groups.

Metodología: Se trata de una revisión integradora que buscó literatura entre los años 2007 a 2022, a través de los descriptores “malaria”, “enfermedades del sistema nervioso” y “Brasil”, investigados en las plataformas LILACS, PUBMED y SciELO. Se utilizaron publicaciones con texto completo disponible, publicadas entre 2007 y 2022, en inglés, portugués y español. Así como también, se excluyeron los estudios que no abordaban la propuesta de investigación, así como los estudios repetidos o duplicados. Resultados: Las manifestaciones neurológicas más frecuentemente reportadas en los estudios presentados se caracterizan por edema cerebral, convulsiones, coma, déficit cognitivo, deterioro del hipocampo y alteración de la conciencia. Además, los signos y síntomas clínicos de gravedad difieren en adultos y niños, y en ambos casos pueden configurar la forma letal de la patología. Conclusión: Las principales manifestaciones neurológicas de la enfermedad incluyen coma, convulsiones, cambios en el tono muscular, pérdida de reflejos y disfunciones conductuales y motoras no especificadas. A pesar de los signos y síntomas clínicos neurológicos encontrados, la malaria cerebral sigue siendo una enfermedad desatendida, con diferencias en el pronóstico entre los grupos de edad de niños y adultos.Objetivo: Compreender as principais manifestações neurológicas da malária cerebral no território brasileiro. Metodologia: Trata-se de uma revisão integrativa que buscou literaturas entre os anos 2007 a 2022, através dos descritores “malária”, “doenças do sistema nervoso” e “Brasil”, pesquisados nas plataformas LILACS, PUBMED e SciELO. Foram utilizadas publicações com texto completo disponível, publicados entre 2007 e 2022, nos idiomas inglês, português e espanhol. Bem como, foram excluídos os estudos que não abordassem a proposta de pesquisa, além de estudos repetidos ou duplicados. Resultados: As manifestações neurológicas mais relatadas nos estudos apresentados caracterizam-se por edema cerebral, convulsões, coma, déficit cognitivo, comprometimento hipocampal e distúrbio da consciência. Outrossim, os sinais e sintomas clínicos de gravidade diferem-se em adultos e crianças, sendo que em ambos os casos podem configurar a forma letal da patologia. Conclusão: As principais manifestações neurológicas da doença envolvem coma, convulsões, alterações do tônus muscular, perda de reflexos, e disfunções comportamentais e motoras sem especificação.
