Preoperative predictors of complex cholecystectomy in videolaparoscopy in a reference service in the state of Pará

Resumo: Objective: to evaluate the preoperative characteristics that predict a complex videolaparoscopic cholecystectomy in the surgical procedure. Methodology: descriptive retrospective observational study through data collection from medical records of patients undergoing cholecystectomy at FSCMPA, which were analyzed through expository tables using Microsoft Word® and Microsoft Excel®. All statistical tests were performed using the Bioestat 5.5 software. Results: There were 114 females in the population (79.2%) and 30 males (20.8%). The most prevalent age group was 30 to 59 years old, with 100 attendances (69.4%). The mean age was 47.59 years. The highest frequency of admission of patients for surgery was elective, constituting 97.2% (140) of cases. Biliary colic was the most prevalent clinical finding and constituted 74.3% (107). The second most prevalent was the presence of cholecystitis 17.4% (25). In the final risk classification, the most prevalent was the score from 0-3, with 75% (108). The chances of having an altered bile duct is 11 times greater in participants at high risk. Conclusion: The results are relevant for the identification of possible predictive preoperative variables, which can contribute to improving the quality of patient care and health management planning.

Metodología: estudio observacional retrospectivo descriptivo mediante la recolección de datos de historias clínicas de pacientes sometidos a colecistectomía en la FSCMPA, los cuales fueron analizados a través de tablas expositivas en Microsoft Word® y Microsoft Excel®. Todas las pruebas estadísticas se realizaron con el software Bioestat 5.5. Resultados: Había 114 mujeres en la población (79,2%) y 30 hombres (20,8%). El grupo de edad más prevalente fue el de 30 a 59 años, con 100 atenciones (69,4%). La edad media fue de 47,59 años. La mayor frecuencia de ingreso de pacientes para cirugía fue electiva, constituyendo el 97,2% (140) de los casos. El cólico biliar fue el hallazgo clínico más prevalente y constituyó el 74,3% (107). El segundo más prevalente fue la presencia de colecistitis 17,4% (25). En la clasificación final de riesgo, la más prevalente fue la puntuación de 0-3, con un 75% (108). Las posibilidades de tener un conducto biliar alterado es 11 veces mayor en los participantes de alto riesgo. Conclusión: Los resultados son relevantes para la identificación de posibles variables preoperatorias predictivas, que pueden contribuir a mejorar la calidad de la atención al paciente y la planificación de la gestión en salud.Objetivo: avaliar as características pré-operatórias que predizem uma colecistectomia videolaparoscópica complexa no ato cirúrgico. Metodologia: estudo observacional retrospectivo descritivo através do levantamento de dados em prontuários de pacientes submetidos a colecistectomia na FSCMPA os quais foram analisados através de tabelas expositivas com o uso do programa Microsoft Word® e Microsoft Excel®. Todos os testes estatísticos foram executados com o auxílio do software Bioestat 5.5. Resultados: O sexo feminino na população foi de 114(79,2%) e do masculino, 30(20,8%). A faixa etária mais prevalente foi a de 30 a 59 anos, com 100 atendimentos (69,4%). A média de idade foi de 47,59 anos. A maior frequência da admissão dos pacientes para a realização da cirurgia foi eletiva, constituindo 97,2 % (140) dos casos. A cólica biliar foi o achado clínico mais prevalente e constituiu 74,3 % (107). O segundo mais prevalente foi a presença de colecistite 17,4% (25). Na classificação do risco final a mais prevalente foi o score de 0-a 3, com 75% (108). As chances de apresentarem ducto biliar alterado é 11 vezes maior nos participantes com risco elevado.
