Personalized therapy in oncology, considering pharmacogenetics and current antineoplastics

Resumo: Humans can express very different therapeutic responses when the same drug is administered. However, the variations may be due to the phenotype, since differences in age, weight, gender, health and diet are among the important factors, but the genotype also contributes to the environment. However, the implementation of pharmacogenetic tests in clinical practice has been slow. There are still many barriers present that lead to its underutilization. The objective of present study is to present a systematic review of meta-analysis on personalized therapy in oncology, considering pharmacogenetics and current antineoplastic agents and as specific objectives: to discuss the development of pharmacogenomics in relation to antineoplastic agents. According to the search strategies, a total of 168 articles were found in the databases selected for the search, with 32 articles in the Pubmed database, 24 in the Lilacs database and 12 in the Scielo database. We conclude that the relationship to the pharmacokinetics and dynamics of drugs used in the treatment of different diseases, may be available in a large database. The benefits of pharmacogenetics range from choosing the drug that has the best therapeutic effects to increasing the effectiveness of clinical trials in discovering new drugs.

Sin embargo, las variaciones pueden deberse al fenotipo, ya que las diferencias en edad, peso, género, salud y dieta se encuentran entre los factores importantes, pero el genotipo también contribuye al medio ambiente. Sin embargo, la implementación de las pruebas farmacogenéticas en la práctica clínica ha sido lenta. Todavía hay muchas barreras presentes que conducen a su subutilización. El objetivo del presente trabajo es presentar una revisión sistemática del metanálisis sobre terapia personalizada en oncología, considerando la farmacogenética y los agentes antineoplásicos actuales y como objetivos específicos: discutir el desarrollo de la farmacogenómica en relación con los agentes antineoplásicos. Según las estrategias de búsqueda, se encontraron un total de 168 artículos en las bases de datos seleccionadas para la búsqueda, con 32 artículos en la base de datos Pubmed, 24 en la base de datos Lilacs y 12 en la base de datos Scielo. Concluimos que la relación con la farmacocinética y la dinámica de los medicamentos utilizados para tratar diferentes enfermedades puede estar disponible en una gran base de datos. Los beneficios de la farmacogenética van desde elegir el medicamento que tiene los mejores efectos terapéuticos hasta aumentar la efectividad de los ensayos clínicos para descubrir nuevos medicamentos.Os humanos podem expressar respostas terapêuticas muito diferentes quando é administrado o mesmo fármaco. No entanto, as variações podem ser devido ao fenótipo, pois, as diferenças na sua idade, peso, género, estado de saúde e dieta são uns dos fatores importantes, mas o genótipo também vem a contribuir com o meio. Contudo a implementação dos testes farmacogenéticos na prática clínica tem sido lenta. Existem ainda muitas barreiras presentes que levam a sua subutilização. O objetivo do presente trabalho é apresentar uma revisão sistemática em meta-analise sobre a terapia personalizada em oncologia, considerando a farmacogenética e os atuais antineoplásicos e como objetivos específicos: discutir sobre o desenvolvimento de farmacogenómica em relação aos antineoplásicos.  De acordo com as estratégias de busca foram encontradas um total de 168 artigos nas bases de dados selecionadas para a busca, sendo 32 artigos na base de dados Pubmed, 24 na base de dados Lilacs e 12 na base de dados Scielo. Concluímos que o relacionamento à farmacocinética e dinâmica de medicamentos utilizados no tratamento de diferentes doenças, possa estar disponível em um grande banco de dados.
