The role of dentists in the diagnosis and treatment of oral condyloma acuminatum in children

Resumo: Papules and warts are clinical manifestations of the oral condyloma acuminatum, which is caused by human papillomavirus types 6 and 11 and transmitted through orogenital contact or autoinoculation. The objective of the present study is to conduct an integrative literature review with  retrospective analytical study selected clinical cases and other research on oral condyloma acuminatum in children published between 2013 and 2023 in the Pubmed, Web of Science, VHL, Google Scholar, Proquest, and ScienceDirect databases, using the descriptors "oral condyloma acuminatum" AND "child abuse" AND "condyloma acuminatum". Eighteen articles were selected after applying inclusion and exclusion criteria. The reported association between condyloma acuminatum in children and sexual abuse varied between 10% to 90%. The lesions were mostly observed in females and usually occurred on oral mucosa and floor, gums, tonsils, and palate. Diagnosis is based on clinical examination, histological observation, and hybridization. Treatment may involve surgical removal with scalpels, laser, cryotherapy, and topical medications such as imiquimod and podophyllotoxin. The role of dentists in the clinical diagnosis and histopathological analysis of oral mucosal lesions must be emphasized.

El objetivo del presente trabajo consiste en realizar una revisión integrativa de la literatura con analítico retrospectivo seleccionó casos clínicos y otras investigaciones sobre condiloma acuminado oral en niños publicados entre 2013 y 2023 en las bases de datos Pubmed, Web of Science, BVS, Google Scholar, Proquest, y ScienceDirect, utilizando los descriptores "condiloma acuminado oral" AND "abuso infantil" AND "condiloma acuminatum". Se seleccionaron dieciocho artículos tras aplicar criterios de inclusión y exclusión. La asociación informada entre el condiloma acuminado en niños y el abuso sexual varió entre 10% y 90%. Las lesiones se observaron principalmente en mujeres y generalmente ocurrieron en la mucosa y el piso de la boca, las encías, las amígdalas y el paladar. El diagnóstico se basa en el examen clínico, la observación histológica, y la hibridación. El tratamiento puede implicar la extirpación quirúrgica con bisturí, láser, crioterapia, y medicamentos tópicos como imiquimod y podofilotoxina. Cabe destacar el papel del odontólogo en el diagnóstico clínico y análisis histopatológico de las lesiones de la mucosa oral.O condiloma acuminado oral representa uma manifestação causada pelo papilomavírus humano (subtipos 6 e 11, especificamente), clinicamente manifestados como formações papulares ou verrucosas, sendo assintomáticas, transmitidas por vias sexual, orogenital ou por autoinoculação. O objetivo do presente trabalho consiste em realizar uma revisão integrativa da literatura com abordagem analítica e retrospectiva com seleção de casos clínicos e pesquisas sobre condiloma acuminado em mucosa oral de crianças, através de um levantamento bibliográfico nas bases de dados Pubmed, Web of Science, BVS, Google Scholar, Proquest e Sciencedirect, utilizando descritores como "Oral Condyloma acuminatum", "Child abuse" e "Condyloma acuminatum" com o operador booleano AND, no período de 2013-2023. A seleção resultou em um total de 18 estudos, após aplicação dos critérios de inclusão e exclusão. Cerca de 10% a 90% dos casos de condiloma acuminado em crianças estão associados a abusos sexuais. As lesões tendem a se manifestar na mucosa oral, gengiva, assoalho da boca, amígdalas e palato, sendo mais comuns no gênero feminino. O diagnóstico é baseado no exame clínico, aspecto histológico e hibridização. O tratamento pode envolver remoção cirúrgica por bisturi, laser, crioterapia e aplicação de agentes tópicos como imiquimod ou podofilotoxina.
