Sinus membrane elevation to increase height in the posterior maxillary region: lateral window technique

Resumo: The reconstruction of the posterior region of atrophic maxillae, aiming at rehabilitation with dental implants, is considered a challenge. Maxillary sinus floor elevation surgery has become essential for bone height gain in this region. The maxillary sinus lift technique was initially described in 1977 and has undergone modifications over the years. Currently, several variations of the original technique have been used to provide access and allow elevation of the sinus membrane. However, the technique by accessing the lateral window is performed on a large scale to allow a safe and efficient path for bone grafting in the region. In planning, in order to avoid complications, the anatomical and systemic knowledge of the patient is crucial. The approach via the lateral window consists of making a bone window in the lateral wall of the maxillary sinus, allowing the lifting of the membrane and the apposition of a biomaterial under it. To obtain success in the technique, it is essential that the patient presents ideal systemic conditions, the professional is aware of their indications and contraindications. In addition, the choice of biomaterial and the surgical time for implant placement can be determining factors for the success of the surgery.

La cirugía de elevación del suelo del seno maxilar se ha vuelto fundamental para ganar altura ósea en esta región. La técnica de elevación de seno maxilar se describió inicialmente en 1977 y ha sufrido modificaciones a lo largo de los años. Actualmente, se han utilizado varias variaciones de la técnica original para proporcionar acceso y permitir la elevación de la membrana del seno. Sin embargo, la técnica de acceder a la ventana lateral se realiza a gran escala para permitir un camino seguro y eficiente para el injerto óseo en la región. En la planificación, para evitar complicaciones, es fundamental el conocimiento anatómico y sistémico del paciente. El abordaje por ventana lateral consiste en realizar una ventana ósea en la pared lateral del seno maxilar, permitiendo el levantamiento de la membrana y la aposición de un biomaterial debajo de la misma. Para obtener éxito en la técnica, es fundamental que el paciente presente condiciones sistémicas ideales, el profesional conozca sus indicaciones y contraindicaciones. Además, la elección del biomaterial y el tiempo quirúrgico para la colocación del implante pueden ser factores determinantes para el éxito de la cirugía.A reconstrução da região posterior de maxilas atróficas, visando a reabilitação com implantes dentários, é considerada um desafio. A cirurgia de elevação do assoalho do seio maxilar tornou-se fundamental para o ganho de altura óssea nessa região. A técnica de levantamento do seio maxilar foi descrita inicialmente em 1977 e ao longo dos anos sofreu modificações. Atualmente, diversas variações da técnica original vêm sendo utilizadas para proporcionar o acesso e permitir a elevação da membrana sinusal. No entanto, a técnica pelo acesso à janela lateral é realizada em larga escala para permitir um caminho seguro e eficiente à enxertia óssea na região. No planejamento, é crucial, a fim de evitar complicações, o conhecimento anatômico e sistêmico do paciente. A abordagem via janela lateral, consiste na confecção de uma janela óssea na parede lateral do seio maxilar, possibilitando o levantamento da membrana e a aposição de um biomaterial sob a mesma. Para obtenção do sucesso na técnica, é fundamental que o paciente apresente condições sistêmicas ideais, o profissional tenha consciência de suas indicações e contraindicações.
