Minimum intervention in the removal of cariated tissue in decidual teeth

Resumo: | Nowadays, traditional restorations on primary teeth have been losing ground to minimally invasive techniques, such as the technique of selective removal of decayed tissue and the technique of preformed metallic crowns from Hall. Once these techniques drastically reduce the risk of the patient needing endodontic treatment, due to accidental or iatrogenic pulp exposure, during the removal of carious dentin in deep caries lesions, this is because the deepest portion of the carious lesion is not removed in the selective removal technique or in the case of Hall technique, in which it does not remove any tissue from the dental structure, maintaining the integrity of the tooth. A qualitative narrative literature review was carried out, of the thematic in focus, using articles available on online platforms, such as VHL salud, Pubmed and Scielo. Priority was given to works published preferably in the period from 2016 to 2020, in Portuguese, English and Chinese After analysis and absorption of reference knowledge, it is concluded that nowadays it is not justified to make use of conventional restoration techniques, since there are already superior alternatives, reported in the literature and also clinically proven, with years of clinical follow-up demonstrating its success. This work aimed to create an aggregate of information for the dental community, presenting a modality of treatment of the decayed tissue of primary teeth. In addition to which microorganisms cause decay, the child's diet, materials used and techniques. |

In addition to which microorganisms cause decay, the child's diet, materials used and techniques.Hoy en día, las restauraciones tradicionales en dientes temporales han ido perdiendo terreno frente a técnicas mínimamente invasivas, como la técnica de extracción selectiva de tejido cariado y la técnica de coronas metálicas preformadas de Hall. Dado que estas técnicas reducen drásticamente el riesgo de que el paciente necesite un tratamiento endodóntico, debido a una exposición pulpar accidental o iatrogénica, durante la extracción de dentina cariosa en lesiones de caries profundas, esto se debe a que la porción más profunda de la lesión cariosa no se elimina en la forma selectiva en la que no remueve ningún tejido de la estructura dental, manteniendo la integridad del diente. Se realizó una revisión bibliográfica narrativa cualitativa, de la temática en foco, utilizando artículos disponibles en plataformas online, como BVS, Pubmed y Scielo. Se dio prioridad a los trabajos publicados preferentemente en el período de 2016 a 2020, en portugués, inglés y chino. Tras el análisis y absorción de los conocimientos de referencia, se concluye que en la actualidad no se justifica hacer uso de técnicas de restauración convencionales, puesto que ya existen alternativas superiores, reportadas en la literatura y también clínicamente probadas, con años de seguimiento clínico demostrando su éxito. Este trabajo tuvo como objetivo realizar un agregado de información para la comunidad odontológica, presentando una modalidad de tratamiento del tejido cariado de los dientes temporales. Además de qué microorganismos provocan la descomposición, la dieta del niño, los materiales utilizados y las técnicas.Nos dias atuais, as restaurações tradicionais em dentes decíduos, vêm perdendo espaço para as técnicas minimamente invasivas, tais como, a técnica de remoção seletiva de tecido cariado e a técnica das coroas metálicas pré-formadas de Hall. Uma vez que essas técnicas, reduzem drasticamente o risco do paciente necessitar de tratamento endodôntico, devido a exposição pulpar acidental ou iatrogênica, durante a remoção de dentina cariada em lesões de cárie profundas, isso porque não se remove a porção mais profunda da lesão cariosa na técnica de remoção seletiva ou no caso de técnica de Hall, em que não remove nenhum tecido da estrutura dental, mantendo a integridade do dente. Foi realizada uma revisão de literatura narrativa qualitativa, da temática em foco, utilizando artigos disponíveis em plataformas onlines, como BVS, Pubmed e Scielo. Foram priorizadas as obras publicadas preferencialmente no período de 2016 a 2020, com língua portuguesa, inglesa e chinesa. Após análise e absorção do conhecimento de referência, conclui-se que nos dias atuais não se justifica fazer uso das técnicas convencionais de restauração, uma vez que já existem alternativas superiores, relatadas na literatura e comprovadas também clinicamente, com anos de acompanhamento clinico demonstrando seu sucesso.
