Effectiveness of pharmacological and non-pharmacological methods of pain management in children during venipuncture: a narrative review

Resumo: | Recent scientific evidence shows that pain associated with acute painful procedures such as venipuncture is an important source of pediatric pain and may have long-term consequences on the behavior and perception of pain in children. The objective is to verify the efficacy of main pharmacological and non-pharmacological methods of pain management during venipuncture in children. This study is a narrative review of an analytical character with research in PUBMED, SciELO and LILACS databases. As the inclusion criteria, only clinical trials from the year 2016 until the data collection data were selected, selected with the descriptors: “pain”, “venipuncture” and “children”. 31 articles were selected that address different types of pain management during venipuncture, 6 referring to pharmacological methods and 25 to non-pharmacological methods. Among the pharmacological ones, the use of dexmedetomidine, Midazolam, a sucrose solution, topical anesthetic, lidocaine and tetracaine hot adhesive and EMLA cream stands out. As for non-pharmacological ones, the main ones are distraction techniques, including playing video games, reading cards, watching cartoons, listening to music, using toys, interacting with parents, inflating balloons, squeezing the ball, coughing, in addition to more current techniques, such as reality virtual and Buzzy system. Studies have sought to elucidate a greater number of non-pharmacological methods in relation to pharmacological ones, as they are less costly and less traumatic. It is important to highlight distraction techniques, which have a variety of applicable methods, especially those related to hearing and vision. |

Por lo tanto, el objetivo es verificar la efectividad de los principales métodos farmacológicos y no farmacológicos de manejo del dolor durante la venopunción en niños. Este estudio es una revisión narrativa analítica con investigaciones en las bases de datos PUBMED, SciELO y LILACS. Como criterios de inclusión, se seleccionaron solo ensayos clínicos desde 2016 a la fecha de recolección de datos, seleccionados con los descriptores: “dolor”, “venopunción” y “niños”. Se seleccionaron 31 artículos que abordan diferentes tipos de manejo del dolor durante la venopunción, de los cuales 6 se refieren a métodos farmacológicos y 25 a métodos no farmacológicos. Entre los farmacológicos destaca el uso de dexmedetomidina, midazolam, solución de sacarosa, anestésico tópico, lidocaína, tetracaína, parche caliente y crema EMLA. En cuanto a las no farmacológicas, las principales son las técnicas de distracción, que incluyen jugar videojuegos, leer cartas, mirar dibujos, escuchar música, usar juguetes, interactuar con los padres, inflar globos, apretar la pelota, toser, además de técnicas como como realidad virtual y sistema Buzzy. Los estudios han buscado dilucidar un mayor número de métodos no farmacológicos en relación a los farmacológicos, por ser menos costosos y menos traumáticos. Es importante destacar las técnicas de distracción, que tienen una variedad de métodos aplicables, principalmente los relacionados con la audición y la visión.Evidências científicas recentes mostram que a dor associada a procedimentos dolorosos agudos, como a punção venosa, é uma importante fonte de dor pediátrica e pode ter consequências a longo prazo no comportamento e na percepção da dor em crianças. Então, objetiva-se verificar a eficácia dos principais métodos farmacológicos e não farmacológicos de manejo da dor durante venipuntura em crianças. Este estudo é uma revisão narrativa de caráter analítico com pesquisas nas bases de dados PUBMED, SciELO e LILACS. Como critério de inclusão, foram selecionados apenas ensaios clínicos do ano de 2016 até a data da coleta de dados, selecionados com os descritores: “dor”, “punção venosa” e “crianças”. Foram selecionados 31 artigos que abordam diferentes tipos de manejo da dor durante a punção venosa, sendo 6 referentes a métodos farmacológicos e 25 a métodos não farmacológicos. Dentre os farmacológicos, destaca-se o uso de dexmedetomidina, Midazolam, solução de sacarose, anestésico tópico, lidocaína, tetracaína, adesivo quente e EMLA creme. Quanto aos não farmacológicos, os principais são as técnicas de distração, incluindo jogar videogame, ler cartas, assistir desenhos, ouvir música, usar brinquedos, interagir com os pais, inflar balões, apertar a bola, tossir, além de técnicas, como realidade virtual e sistema Buzzy. Estudos têm buscado elucidar um maior número de métodos não farmacológicos em relação aos farmacológicos, por serem menos onerosos e menos traumáticos.
