Polycystic ovary syndrome (PCOS), pathophysiology and treatment, a review

Resumo: Polycystic ovary syndrome is a pathology of multifactorial etiology and one of the main causes of female infertility worldwide. In addition to changes in reproductive physiology, such as irregular menstrual cycles, anovulation, hyperandrogenism and the presence of multiple ovarian cysts, metabolic changes also occur that can affect the quality of life and survival of women with the syndrome. Endocrine impairment linked to polycystic ovary syndrome occurs due to the development of metabolic syndrome, a condition characterized by impaired glucose tolerance, dyslipidemia and hypertension. However, the manifestation of symptoms associated with polycystic ovary syndrome depends on the interaction between genetic and epigenetic factors, lifestyle and eating habits. The interrelationship between these factors results in the phenotypic diversity presented by women with the syndrome. That is, the clinical manifestations and symptomatic severity vary among the carrier women, so that the treatment is individualized for each patient. The therapeutic management of the syndrome is aimed at symptomatic relief and ovulation induction, since there is still no curative treatment. This narrative review aims to highlight the main clinical, endocrine and metabolic aspects of polycystic ovary syndrome, one of the main causes of female infertility.

Además de los cambios en la fisiología reproductiva, como ciclos menstruales irregulares, anovulación, hiperandrogenismo y la presencia de múltiples quistes ováricos, también ocurren cambios metabólicos que pueden afectar la calidad de vida y supervivencia de las mujeres con el síndrome. El deterioro endocrino relacionado con el síndrome de ovario poliquístico se produce debido al desarrollo del síndrome metabólico, una condición caracterizada por alteración de la tolerancia a la glucosa, dislipidemia e hipertensión. Sin embargo, la manifestación de los síntomas asociados al síndrome de ovario poliquístico depende de la interacción entre factores genéticos y epigenéticos, estilo de vida y hábitos alimentarios. La interrelación entre estos factores da como resultado la diversidad fenotípica que presentan las mujeres con el síndrome. Es decir, las manifestaciones clínicas y la gravedad sintomática varían entre las mujeres portadoras, por lo que el tratamiento es individualizado para cada paciente. El manejo terapéutico del síndrome está dirigido al alivio sintomático y la inducción de la ovulación, ya que aún no existe un tratamiento curativo. Esta revisión narrativa pretende resaltar los principales aspectos clínicos, endocrinos y metabólicos del síndrome de ovario poliquístico, una de las principales causas de infertilidad femenina.A síndrome dos ovários policísticos é uma patologia de etiologia multifatorial e uma das principais causas de infertilidade feminina mundialmente. Além de alterações na fisiologia reprodutiva, como ciclos menstruais irregulares, anovulação, hiperandrogenismo e presença de múltiplos cistos no ovário, também ocorrem alterações metabólicas capazes de afetar a qualidade de vida e a sobrevida de mulheres portadoras da síndrome. O comprometimento endócrino atrelado à síndrome dos ovários policísticos ocorre devido ao desenvolvimento de síndrome metabólica, condição caracterizada por tolerância à glicose diminuída, dislipidemia e hipertensão. Contudo, a manifestação dos sintomas associados à síndrome dos ovários policísticos depende da interação entre fatores genéticos, epigenéticos, estilo de vida e hábitos alimentares. A inter-relação entre esses fatores resulta na diversidade fenotípica apresentada por mulheres portadoras da síndrome. Ou seja, as manifestações clínicas e a gravidade sintomática variam entre as mulheres portadoras, de forma que o tratamento é individualizado para cada paciente. O manejo terapêutico da síndrome é voltado ao alívio sintomático e indução da ovulação, uma vez que ainda não existe um tratamento curativo.
