Depressive symptoms and serum levels of vitamins B12 and folate in elderly from Florianópolis/SC: Epifloripa Elderly Study

Resumo: | Depression is characterized by symptoms of deep sadness and mood swings. Deficient levels of folate and vitamin B12 have been related to the development of depression, as they play key roles in the proper functioning of the brain. The aim of the study was to investigate the association between the serum concentration of vitamin B12 and folate and symptoms of depression in elderly people in southern Brazil. Data from the second wave of the population-based and household survey (EpiFloripa) were analyzed. A total of 590 elderly (≥60 years) participated in the collection of blood for biochemical analysis. The presence of symptoms of depression was assessed using the Geriatric Depression Scale (GDS-15) with a cut-off point ≥ 6. Vitamins B12 and folate were measured through biochemical tests. Binary Logistic Regression was used to analyze the association between the variable symptoms of depression and vitamins. 16.4% of the elderly had symptoms of depression, being more frequent in women, people with chronic disease and low education. The lower mean serum vitamin B12 does not explain the presence of symptoms of depression (OR= 1.000; CI: 0.999; 1.001). The lowest mean serum folate was associated in the crude analysis with the presence of symptoms of depression (OR= 0.931; CI: 0.886; 0.978) and was maintained after adjustment for control variables (OR= 0.937; CI: 0.888; 0.988). The highest mean serum folate is a protective factor for the occurrence of depressive symptoms. Although vitamin B12 has not been associated with symptoms of depression, maintenance of higher averages is recommended in the elderly. |

Los niveles deficientes de folato y vitamina B12 se han relacionado con el desarrollo de la depresión, ya que juegan un papel clave en el buen funcionamiento del cerebro. El objetivo del estudio fue investigar la asociación entre la concentración sérica de vitamina B12 y folato y los síntomas de depresión en ancianos del sur de Brasil. Se analizaron los datos de la segunda ola de la encuesta de población y de hogares (EpiFloripa). Un total de 590 ancianos (≥60 años) participaron de la colecta de sangre para análisis bioquímico. La presencia de síntomas de depresión se evaluó mediante la Escala de Depresión Geriátrica (GDS-15) con un punto de corte ≥ 6. Las vitaminas B12 y los folatos se midieron mediante pruebas bioquímicas. Se utilizó Regresión Logística Binaria para analizar la asociación entre la variable síntomas de depresión y vitaminas. El 16,4% de los ancianos presentó síntomas de depresión, siendo más frecuente en mujeres, personas con enfermedades crónicas y baja escolaridad. La media más baja de vitamina B12 sérica no explica la presencia de síntomas de depresión (OR= 1.000; IC: 0.999; 1.001). El folato sérico medio más bajo se asoció en el análisis crudo con la presencia de síntomas de depresión (OR= 0,931; IC: 0,886; 0,978) y se mantuvo después del ajuste por variables de control (OR= 0,937; IC: 0,888; 0,988). El folato sérico medio más alto es un factor protector para la aparición de síntomas depresivos. Aunque la vitamina B12 no se ha asociado con síntomas de depresión, se recomienda el mantenimiento de promedios más altos en los ancianos.A depressão caracteriza-se por sintomas de tristeza profunda e alteração no humor. A presença de sintomas de depressão foi avaliada pela Escala Geriátrica de Depressão (GDS-15) com ponto de corte ≥ 6. As vitaminas B12 e o folato, foram mensuradas através de exames bioquímicos. Foi utilizada a Regressão Logística Binária para as análises de associação entre a variável sintomas de depressão e as vitaminas. 16,4% dos idosos apresentaram sintomas de depressão, sendo mais frequente em mulheres, pessoas com doença crônica e baixa escolaridade. A menor média da vitamina B12 sérica não explica a presença de sintomas de depressão (OR= 1,000; IC: 0,999; 1,001). A menor média de folato sérico associou-se na análise bruta à presença de sintomas de depressão (OR= 0,931; IC: 0,886; 0,978) e foi mantida após o ajuste para as variáveis de controle (OR= 0,937; IC: 0,888; 0,988). A maior média de folato sérico é um fator de proteção para a ocorrência de sintomas depressivos.
