Manifestation of Histoplasmosis in the oral cavity: case report

Resumo: Background: Histoplasmosis is a systemic fungal infection caused by the dimorphic fungus Histoplasma Capsulatum which has spores in soils, mainly acids, in endemic areas. Clinical manifestations are associated with immunocompetence and present signs and symptoms that include: fever, weakness, weight loss, respiratory signs, hepatosplenomegaly, superficial lymphadenopathies. Oral manifestations are secondary in immunocompetent patients, but in immunosuppressed patients, especially those infected with HIV, the disease becomes symptomatic in 95% of cases where the CD4 count is less than 200/mm³. Oral symptoms present as everted edge ulcerative lesion, nodules or even vegetative lesions which, although rare, deserve careful observation to be early diagnosed and treated to avoid systemic complications. Objective and Case Report: The objective of this work is to report the diagnosis of the oral manifestation of Histoplasmosis, in an immunosuppressed patient, with an ulcerative lesion in the upper lip with characteristics of malignancy. Conclusion: The isolated oral manifestations of Histoplasmosis make the diagnosis more difficult due to its rarity, within this context, it is essential to know the oral manifestations of Histoplasmosis, especially in immunocompromised patients, who are more susceptible to opportunistic infections.

Las manifestaciones clínicas se asocian con la inmunocompetencia y muestran signos y síntomas como: fiebre, debilidad, pérdida de peso, signos respiratorios, hepatoesplenomegalia, linfadenopatías superficiales. Las manifestaciones orales son secundarias en los pacientes inmunocompetentes, pero en los pacientes inmunodeprimidos, especialmente los infectados por el VIH, la enfermedad se vuelve sintomática en el 95% de los casos en que el recuento de CD4 es inferior a 200/mm³. Los síntomas orales se presentan como una lesión ulcerosa de borde evertido, nódulos o incluso lesiones vegetativas que, aunque raras, merecen una observación cuidadosa para ser diagnosticadas y tratadas a tiempo para evitar complicaciones sistémicas. Objetivo e informe de caso: El objetivo de este estudio es informar sobre el diagnóstico de la manifestación oral de la histoplasmosis, en un paciente inmunodeprimido, con una lesión ulcerosa del labio superior con características de malignidad. Conclusión: Las manifestaciones orales de la histoplasmosis en forma aislada dificultan el diagnóstico debido a su rareza; en este contexto, el conocimiento de las manifestaciones orales de la histoplasmosis es esencial, especialmente en los pacientes inmunocomprometidos, que son más susceptibles a las infecciones oportunistas.Fundamento: A histoplasmose é uma infecção fúngica sistêmica causada pelo fungo dimórfico Histoplasma Capsulatum o qual possui esporos em solos, principalmente ácidos, em áreas endêmicas. Manifestações clinicas estão associadas a imunocompetência e apresentam sinais e sintomas que incluem: febre, fraqueza, perda de peso, sinais respiratórios, hepatoesplenomegalia e linfadenopatias superficiais. Manifestações orais são secundarias em pacientes imunocompetentes, porém em pacientes imunossuprimidos, especialmente os infectados por HIV, a doença passa a ser sintomática em 95% dos casos onde a contagem de CD4 é inferior a 200/mm³. Os sinais clínicos orais apresentam-se como lesão ulcerativa de borda evertida, nódulos ou até mesmo lesões vegetativas. Apesar de raras, merecem cuidadosa observação para que sejam precocemente diagnosticadas e tratadas para evitar complicações sistêmicas. Objetivo e Relato de Caso: O objetivo desse trabalho é relatar o diagnóstico da manifestação oral de Histoplasmose, em um paciente imunossuprimido, com uma lesão ulcerativa em lábio superior com características de malignidade.
