Surgical treatment of comminuted mandible fracture involving simplification with miniplates: Report of two cases

Resumo: Comminuted mandible fractures (CMF) are fractures that have multiple fracture lines resulting in several bone fragments. Among the forms of treatment for this type of fracture, open reduction with rigid internal fixation (RIF) seems to be the best option. This paper aims to report two cases of comminuted mandibular fracture treated by open reduction, simplification, and rigid internal fixation. In both cases, the fracture was simplified with 1.5mm miniplates and after the anatomy was restored, a 2.4mm reconstruction plate and bicortical anchor screws were installed. This type of approach provides stable occlusion, reconstruction of the mandibular anatomy and the absence of loose fragments that could progress to infection. It is concluded that the open reduction and the RIF of comminuted fractures using the principle of simplification optimizes the results and reduces complications.

Entre las formas de tratamiento para este tipo de fracturas, la reducción abierta con fijación interna rígida (FIR) parece ser la mejor opción. Este artículo tiene como objetivo reportar dos casos de fractura mandibular conminuta tratados mediante reducción abierta, simplificación y fijación interna rígida. En ambos casos se simplificó la fractura con miniplacas de 1,5 mm y, una vez restaurada la anatomía, se instalaron una placa de reconstrucción de 2,4 mm y tornillos de anclaje bicorticales. Este tipo de abordaje proporciona una oclusión estable, reconstrucción de la anatomía mandibular y la ausencia de fragmentos sueltos que puedan progresar a una infección. Se concluye que la reducción abierta y la FIR de las fracturas conminutas mediante el principio de simplificación optimiza los resultados y reduce las complicaciones.As fraturas cominutivas da mandíbula (FCM) são fraturas que apresentam múltiplas linhas de fraturas resultando em vários fragmentos ósseos. Entre as formas de tratamento para este tipo de fratura, a redução aberta com a fixação interna rígida (FIR) parece ser a melhor opção. O presente trabalho tem como objetivo relatar dois casos de fratura mandibular cominutiva tratadas por redução aberta, simplificação e fixação internar rígida. Nos dois casos a fratura foi simplificada com miniplacas 1.5mm e após o restabelecimento da anatomia foi instalada uma placa de reconstrução 2.4mm e parafusos de ancoragem bicortical. Esse tipo de abordagem fornece uma oclusão estável, a reconstrução da anatomia mandibular e ausência de fragmentos soltos que pudessem evoluir para infecção.
