Coverage of doses of the HPV vaccine and variation by level of material deprivation in brazilian municipalities, 2012 to 2018

Resumo: Vaccination against HPV is an important prevention strategy that can reduce the burden of disease and the incidence of cancers in the long term. Therefore, this study aimed to characterize the temporal trend of HPV vaccine dose coverage and the variation in the level of material deprivation of Brazilian municipalities between 2012 and 2018. This is a longitudinal observational quantitative study that combined analysis of several observation units with a temporal trend design, which used data from the Sistema de Informação de Agravos de Notificação (SINAN). The frequencies of doses applied against the HPV vaccine were presented by year of analysis for the total population and stratified by sex. Municipalities were divided into quintiles according to the deprivation score of the Brazilian Deprivation Index. The frequencies of vaccine doses administered by deprivation quintiles were presented for the municipalities year by year. A total of 34,268,998 doses of HPV vaccine were applied in Brazil during the analyzed period, with higher doses during the national campaign period. Higher doses were observed among women compared to men (p=0.025). There was a trend of reduction in the number of doses over the years. Therefore, there is a need to develop measures to increase vaccination coverage against HPV, such as health education, sex education and public policies to reduce health and socioeconomic inequities.

Consiguiente, el objetivo de este estudio fue caracterizar la tendencia temporal de la cobertura de dosis de la vacuna HPV y la variación por nivel de privación material de los municipios brasileños, entre 2012 y 2018. Se trata de un estudio cuantitativo observacional longitudinal, que combinó un análisis de varias unidades de observación con un diseño de tendencia temporal, que utilizó datos del Sistema de Informação de Agravos de Notificação (SINAN). Las frecuencias de dosis aplicadas contra la vacuna HPV se presentaron por año de análisis, para la población total y estratificadas por sexo. Los municipios fueron divididos en quintiles de acuerdo con el puntaje de privación del Índice Brasileño de Privación. Se presentaron las frecuencias de las dosis de vacunas administradas por quintiles de privación para los municipios por año. Un total de 34.268.998 dosis de vacuna HPV fueron aplicadas en Brasil, con mayor número de dosis durante la campaña nacional. Se observaron mayores números de dosis entre las mujeres en comparación con los hombres (p = 0,025). Hubo una tendencia de reducción en el número de dosis a lo largo de los años. Luego, existe la necesidad de desarrollar medidas dirigidas a aumentar las coberturas de vacunación contra el HPV, tales como educación en salud, educación sexual y políticas públicas dirigidas a reducir las inequidades en salud y socioeconómicas.A vacinação contra o HPV é uma importante estratégia de prevenção que pode reduzir a carga da doença e a incidência de cânceres a longo prazo. Assim, o objetivo deste estudo foi caracterizar a cobertura das doses de HPV em adolescentes brasileiros e seus padrões de variação de acordo com as características geográficas e o nível de privação material dos municípios, entre 2012 e 2018. Trata-se de um estudo transversal, que utilizou dados do Sistema de Informação do Programa Nacional de Imunização (SI-PNI), para os 5.570 municípios brasileiros que tiveram registro de doses aplicadas de vacinas contra o HPV nos anos de interesse. As frequências de doses aplicadas de vacina contra o HPV foram apresentadas por ano de análise, para a população total e estratificadas por sexo. Os municípios foram divididos em quintis segundo pontuação de privação do Índice Brasileiro de Privação. As frequências de doses de vacinas aplicadas por quintis de privação foram apresentadas para os municípios ano a ano. Um total de 34.268.998 doses de vacina contra o HPV foram aplicadas no Brasil entre o período, com maiores números de doses durante o período da campanha nacional. Maiores números de doses foram observados entre mulheres comparadas a homens (p=0,025). Houve uma tendência de redução do número de doses ao longo dos anos.
