Diagnóstico tardio da degeneração mixomatosa valvar mitral e a obesidade e taquicardia como cofatores complicadores de prognóstico – Relato de caso

Resumo: Myxomatous mitral valve degeneration is among the most common heart diseases in dogs, causing changes in the heart valves that degenerate and progress to atrial ventricular overload. This disease can be treated according to the stage of the disease, ranging from A to D, with subdivisions in stage B to B1 and B2. In this case report we present the late diagnosis of an elderly female dog in stage C, and obesity and tachycardia as complicating cofactors of the disease, will be addressed. Obesity is characterized by the accumulation of adipose tissue, it is associated with genetic and hormonal factors and metabolic disorders that can generate cardiac dysfunction, however, the heart dilates, requires greater effort, increases the heart rate and it becomes impracticable to maintain this HR for a prolonged period. An 8 years-old Shih Tzu female dog, weighing 10,3 Kg, was seen in private practice in a city in the state of Goiás. On clinical examination, she was dyspneic and apathetic, RR 48 mrm, HR 240 bpm, on cardiac auscultation she had a grave V murmur. The echocardiogram showed advanced myxomatous mitral valve degeneration and pulmonary edema. Therapy with furosemide, pimobendan, atenolol, amlodipine and benazepreil, was started, but the patient died. This report aims to show the importance of early diagnosis of the disease and obesity and tachycardia as complicating prognostic cofactors. The dog had a quick diagnosis, but without a timely and desirable drug effect. The necropsy was not authorized for further studies.

The necropsy was not authorized for further studies.  La degeneración mixomatosa de la válvula mitral se encuentra entre las enfermedades cardíacas más comunes en los perros y provoca cambios en las válvulas cardíacas que degeneram y progressan a una sobrecarga atrioventricular. Esta enfermedad se puede tratar según el estadio de la enfermedad, que va de A a D, con subdivisiones en estadio B a B1 y B2. En este reporte de caso, se abordará el diagnóstico tardío de una perra anciana em estágio C y la obesidad y la taquicardia como factores que complican la enfermedad. La obesidad se caracteriza por lá acumulación de tejido adiposo, se associa a factores genéticos, hormonales y transtornos metabólicos que pueden generar disfunción cardíaca, sin embargo, el corazón se dilata, requiere mayor esfuerzo, aumenta la frecuencia cardiaca y se hace impracticable mantener essa FC por un tiempo prolongado período. Una perra Shih Tzu de 8 años, con un peso de 10,3 Kg, fue vista en una práctica privada en una ciudad del estado de Goiás. Al examen clínico se encontraba disneica y apatica, FR 48 rpm, FC 240 Ipm, a lá auscultación cardiaca soplo grado V. El ecocardiograma mostró Degeneración mixomatosa avanzada de la válvula mitral y edema pulmonar. Se inició terapia con furosemida, pimobendam, atenolol, amlodipino y benazepril, pero el paciente falleció. Este informe tiene como objetivo mostrar la importancia del diagnóstico precoz de la enfermedad y la obesidad y la taquicardia como cofactores pronosticos de complicación. El perro tuvo un diagnostico rápido, pero sin un efecto farmacológico oportuno y deseable, no se autorizó la necropsia para estudios posteriores. A degeneração mixomatosa valvar mitral está entre as doenças cardíacas mais comuns em cães, causando alteração nas valvas do coração que se degeneram e progridem para sobrecarga atrioventricular. Essa enfermidade pode ser tratada de acordo com o estágio da doença, variando entre A a D, com subdivisões no estágio B para B1 e B2. Nesse relato de caso, será abordado o diagnóstico tardio de uma cadela idosa no estágio C e como cofatores complicadores da doença a obesidade e a taquicardia. A obesidade é caracterizada pelo acúmulo de tecido adiposo, está associada a fatores genéticos, hormonais e distúrbios metabólicos que podem gerar disfunção cardíaca, contudo, o coração dilata, exige maior esforço, aumenta a frequência cardíaca e se torna inviável manter essa FC por período prolongado. Uma cadela de 8 anos de idade, da raça Shih Tzu, peso 10,3 Kg, foi atendida em consultório particular, em uma cidade do estado de Goiás. Em exame clínico apresentava-se dispnéica e apática, FR 48 mrm, FC 240 bpm, na ausculta cardíaca apresentava sopro grau V. O ecocardiograma apresentou degeneração mixomatosa valvar mitral avançada e edema pulmonar.  Iniciado a terapia com furosemida, pimobendam, atenolol, anlodipino e benazepril, mas a paciente veio a óbito. Este relato tem o objetivo de mostrar a importância do diagnóstico precoce da doença e como cofatores complicadores de prognóstico a obesidade e a taquicardia. A cadela teve o diagnóstico rápido, mas sem tempo hábil de efeito desejável medicamentoso.
