Bacterial resistance to antibiotics used in treatment of gonorreia: a literature review

Resumo: Gonorrhea is a sexually transmitted infections that is among the most common public health problems worldwide. This disease is caused by the gram-negative bacterium Neisseria gonorrhoeae, which is a microorganism restricted to humans. Since it presents itself as a frequent infection, N. gonorrhoeae shows an extraordinary ability to develop resistance to multiple classes of antibiotics, becoming extremely important, besides worrying public health authorities. Therefore, the objective of this work was to present a survey about the main antibiotics associated with resistance attributed to N. gonorrhoeae, according to scientific publications presented in 2017 and 2018, in different databases (Scielo, PubMed, Lilacs and Google Scholar). Through an integrative literature review, using terms according to the Health Descriptors - DeCS, articles were found based on national and international data that had a close relationship with the proposed theme. They were selected for data analysis and description, thus allowing an understanding of the problem presented. Antibiotic resistance by N. gonorrhoeae strains is found to be widespread worldwide. It is clear that ceftriaxone, cefixime, cefotaxime, penicillins, tetracyclines and macrolide derivatives are no longer effective in treating infections caused by this microorganism. Findings warn of this serious public health problem.

Esta enfermedad es causada por la bacteria gram-negativa Neisseria gonorrhoeae, que es un microorganismo restringido a los humanos. Al presentarse como una infección frecuente, N. gonorrhoeae muestra una extraordinaria capacidad para desarrollar resistencia a múltiples clases de antibióticos, volviéndose de suma importancia, además de preocupar a las autoridades de salud pública. Por tanto, el objetivo de este trabajo fue presentar una encuesta sobre los principales antibióticos asociados a la resistencia atribuida a N. gonorrhoeae, según publicaciones científicas presentadas en los años 2017 y 2018, en diferentes bases de datos (Scielo, PubMed, Lilacs y Google Académico). Mediante una revisión integradora de la literatura, utilizando términos de acuerdo a los Descriptores de Salud - DeCS, se localizaron en bases de datos nacionales e internacionales artículos que tuvieran una estrecha relación con la temática propuesta. Fueron seleccionados para el análisis y descripción de los datos, lo que permitió comprender el problema presentado. Se concluyó que la resistencia a los antibióticos, por parte de cepas de N. gonorrhoeae, se está extendiendo por todo el mundo. Es posible darse cuenta de que los antibióticos ceftriaxona, cefixima, cefotaxima, penicilinas, tetraciclinas y derivados de macrólidos ya no son efectivos en el tratamiento de infecciones causadas por este microorganismo. Los hallazgos advierten de este grave problema de salud pública.A gonorreia é uma doença sexualmente transmissível que está entre os problemas de saúde pública mais comum em todo o mundo. Essa doença tem como agente etiológico a bactéria gram-negativa Neisseria gonorrhoeae que é um microrganismo restrito aos seres humanos. Por se apresentar como uma infecção frequente, N. gonorrhoeae evidencia, uma extraordinária capacidade de desenvolver resistência a múltiplas classes de antibióticos, tornando-se de extrema importância, além de preocupar as autoridades de saúde pública. Diante disso, o objetivo deste trabalho foi apresentar um levantamento acerca dos principais antibióticos associados à resistência atribuída a N. gonorrhoeae, de acordo com publicações científicas apresentadas nos anos de 2017 e 2018, em diferentes bases de dados (Scielo, PubMed, Lilacs e Google Acadêmico). Por meio de uma revisão integrativa da literatura, utilizando termos de acordo com os Descritores em Saúde – DeCS localizou-se em base de dados nacionais e internacionais, artigos que possuíssem íntima relação com o tema proposto. Os mesmos foram selecionados para a análise e descrição dos dados, permitindo assim uma compreensão do problema apresentado. Concluiu-se que a resistência aos antibióticos, por linhagens de N. gonorrhoeae, se alastra no mundo. É possível perceber que antibióticos ceftriaxona, cefixima, cefotaxima, penicilinas, tetraciclinas e derivados dos macrolídeos já não são mais eficazes no tratamento de infecções provocadas por este microrganismo.
