Evaluation of diabetic retinopathy in adults with type 1 diabetes at Ceará state

Resumo: It is estimated that there are more than 30 thousand people with type 1 diabetes mellitus (DM1) in Brazil. Among the complications, diabetic retinopathy (DR) is one of the most prevalent, affecting up to 50% of DM1 patients in some populations. This study aims to estimate the prevalence of DR in DM1 or Latent autoimmune diabetes in adults (LADA), in addition to assessing risk factors involved. A cross-sectional study was carried out evaluating 81 patients over 18 years of age and diagnosed with long-term type 1 DM. Demographic, clinical and laboratory aspects were assessed using a structured questionnaire. Ophthalmological examination data were collected from medical records. We observed a 33% prevalence of DR in patients with DM1 in the state of Ceará. This complication was more associated with older patients (p = 0.004), longer DM1 time (p <0.001), inadequate metabolic control and, mainly, increased creatinine levels (p = 0.011). It was observed that patients with episodes of severe hypoglycemia had a higher prevalence of DR. In addition, an important association was observed between DR and diabetic nephropathy, as described in the literature. This study demonstrates the high prevalence of DR in patients with DM1 and confirms the need for tracking the disease, especially in patients with longer disease duration and inadequate metabolic control and other complications such as nephropathy.

Entre las complicaciones, la retinopatía diabética (RD) es una de las más prevalentes, afectando hasta al 50% de los pacientes con DM1 en algunas poblaciones. Este estudio tiene como objetivo estimar la prevalencia de RD en DM1 o Diabetes autoinmune latente en adultos (LADA), además de evaluar los factores de riesgo involucrados. Se realizó un estudio transversal en el que se evaluaron 81 pacientes mayores de 18 años diagnosticados de DM tipo 1 de larga evolución. Los aspectos demográficos, clínicos y de laboratorio se evaluaron mediante un cuestionario estructurado. Los datos del examen oftalmológico se obtuvieron de los registros médicos. Observamos una prevalencia del 33% de RD en pacientes con DM1 en el estado de Ceará. Esta complicación se asoció más con pacientes de mayor edad (p = 0,004), mayor tiempo de DM1 (p <0,001), control metabólico inadecuado y, principalmente, niveles elevados de creatinina (p = 0,011). Se observó que los pacientes con episodios de hipoglucemia severa tenían una mayor prevalencia de RD. Además, se observó una asociación importante entre la ER y la nefropatía diabética, como se describe en la literatura. Este estudio demuestra la alta prevalencia de RD en pacientes con DM1 y confirma la necesidad de realizar un seguimiento de la enfermedad, especialmente en pacientes con mayor duración de la enfermedad y control metabólico inadecuado y otras complicaciones como la nefropatía.Estima-se que existam mais de 30 mil pessoas com diabetes mellitus tipo 1 (DM1) no Brasil. Foi realizado um estudo de corte-transversal avaliando 81 pacientes com mais de 18 anos e diagnosticados com DM tipo 1 de longa evolução. Aspectos demográficos, clínicos e laboratoriais foram avaliados através de questionário estruturado. Dados do exame oftalmológico foram coletados de prontuários. Observamos uma prevalência de 33% de RD em pacientes portadores de DM1 no estado do Ceará. Tal complicação foi mais associada a doentes com maior idade (p=0,004), maior tempo de DM1 (p<0,001), controle metabólico inadequado e principalmente, níveis aumentados de creatinina (p=0,011). Foi observado que pacientes com episódios de hipoglicemia grave apresentavam maior prevalência de RD. Além disso, foi observado uma associação importante entre RD e nefropatia diabética, conforme o descrito na literatura.
