Risk factors for maternal mortality during the pandemic of COVID-19

Resumo: Maternal mortality is an important indicator that reflects public health problems, as well as conditions of determinants and conditioning factors in health. With the advent of the pandemic of COVID-19, vulnerable populations and individuals became susceptible to the disease, among them pregnant women, who became a group of great concern, given the evolution of maternal death at extremely high levels associated with COVID-19. In this sense, this study aims to identify the risk factors associated with maternal mortality during the pandemic of COVID-19. This is a cross-sectional study, with a descriptive approach, of the integrative literature review (ILR) type. For data collection strategy, the bibliographic databases were used: PubMed, LILACS and SciELO, using inclusion and exclusion criteria to select the publications. The results showed that several factors can influence maternal mortality, some directly and others indirectly. The signs and symptoms of COVID-19 were an important predictor for the most severe cases, besides being strongly associated with those pregnant women who have some comorbidity, who needed hospitalization or even mechanical ventilation, as well as age, pregnant women from low and middle income countries, among other characteristics linked to maternal mortality. In this sense, it is concluded that there is a need for more attention to the health conditions of pregnant women, both in the sociopolitical context as well as in public policies, in order to minimize the inequities and weaknesses observed in this population.

Con la llegada de la pandemia de COVID-19, las poblaciones e individuos vulnerables se volvieron susceptibles a la enfermedad, entre ellos las mujeres embarazadas, que se convirtieron en un grupo de gran preocupación, dada la evolución de la muerte materna en niveles extremadamente altos asociados a COVID-19. En este sentido, este estudio pretende identificar los factores de riesgo asociados a la mortalidad materna durante la pandemia de COVID-19. Se trata de un estudio transversal, con abordaje descriptivo, del tipo revisión integradora de la literatura (RIL). Para la estrategia de coleta de datos, se utilizaron las bases de datos bibliográficas: PubMed, LILACS y SciELO, utilizando criterios de inclusión y exclusión para la selección de las publicaciones. Los resultados muestran que diversos factores pueden influir en la mortalidad materna, algunos de forma directa y otros de forma indirecta. Los signos y síntomas del COVID-19 fueron un importante predictor para los casos más graves, además de sufrir una fuerte asociación con aquellas embarazadas que tienen alguna comorbilidad, que necesitaron hospitalización o incluso ventilación mecánica, así como la edad, las embarazadas de países de renta baja y media, entre otras características vinculadas a la mortalidad materna. En este sentido, se concluye que es necesaria una mayor atención a las condiciones de salud de las mujeres embarazadas, tanto en el contexto sociopolítico como en las políticas públicas, para minimizar las inequidades y debilidades observadas en esta población.A mortalidade materna é importante indicador que refletem os problemas de saúde pública, bem como condições de determinantes e condicionantes em saúde. Nesse sentido, este estudo tem como objetivo identificar os fatores de risco associados a mortalidade materna durante a pandemia da COVID-19. Os sinais e sintomas da COVID-19 foi um importante preditor para os casos mais graves, além de sofrerem forte associação com àquelas mulheres grávidas que possuem alguma comorbidade, que necessitaram de internação ou mesmo de ventilação mecânica, como também a idade, gestantes de países de baixa e média renda, dentre outras características ligadas a mortalidade materna.
