Clinical and perinatal outcomes of unvaccinated pregnant women with and without Covid-19 undergoing cesarean delivery: a retrospective cohort study

Resumo: | Objective: To describe the clinical and perinatal outcomes of unvaccinated pregnant women with Covid-19 who underwent cesarean delivery in a reference maternity hospital in southern Brazil, and compare them with those of unvaccinated pregnant women with similar characteristics, without Covid-19 infection. 19. Methods: This was a retrospective cohort study, carried out between December 2021 and August 2022. The studied population consisted of pregnant women with Covid-19 who were not immunized with the vaccine against Covid-19 (n=91), compared to a control group of unvaccinated pregnant women without evidence of previous or current infection with Covid-19 (n= 57). The tests used for statistical analysis were: t test for unpaired independent samples, Mann-Whitney test, chi-square test and two-tailed Fisher's exact test with Freeman-Halton extension. As outcomes, ICU admission and deaths of pregnant women and newborns in both groups were investigated. Results: there was a greater tendency towards ICU admission and death in pregnant women with Covid-19 (RR= 9.4; 95% CI: 0.55 – 162.5) and (RR = 3.15; 95% CI: 0.15 – 64.50), respectively. Newborns had a higher risk of admission to the Neonatal ICU (RR = 1.73; 95% CI: 0.73 – 4.14) and a greater chance of death (RR = 2.45, 95% CI: 0.28 – 21.40). Conclusions: pregnant women with Covid-19 and their newborns had a higher risk of hospitalization in the ICU/Neonatal ICU and a higher risk of death. |

Métodos: Este fue un estudio de cohorte retrospectivo, realizado entre diciembre de 2021 y agosto de 2022. La población estudiada estuvo conformada por gestantes con Covid-19 que no fueron inmunizadas con la vacuna contra el Covid-19 (n=91), frente a un grupo control de gestantes no vacunadas sin evidencia de infección previa o actual por Covid-19 (n = 57). Las pruebas utilizadas para el análisis estadístico fueron: prueba t para muestras independientes no apareadas, prueba de Mann-Whitney, prueba de chi-cuadrado y prueba exacta de Fisher de dos colas con extensión de Freeman-Halton. Como desenlaces se investigó el ingreso en la UCI y la muerte de gestantes y recién nacidos en ambos grupos. Resultados: hubo mayor tendencia al ingreso en UCI y muerte en gestantes con Covid-19 (RR= 9,4; IC 95%: 0,55 – 162,5) y (RR = 3,15; IC 95%: 0,15 – 64,50), respectivamente. Los recién nacidos tuvieron mayor riesgo de ingreso a UCI Neonatal (RR = 1,73; IC 95%: 0,73 – 4,14) y mayor probabilidad de muerte (RR = 2,45, IC 95%: 0,28 – 21,40). Conclusiones: las gestantes con Covid-19 y sus recién nacidos tuvieron mayor riesgo de hospitalización en UCI/UCI Neonatal y mayor riesgo de muerte.Objetivo: Descrever os desfechos clínicos e perinatais de gestantes com Covid-19 não vacinadas submetidas a parto cesariano em uma maternidade de referência do Sul do Brasil, e compará-los com os de gestantes não vacinadas, de características similares, sem infecção por Covid-19. Métodos: Tratou-se de um estudo de coorte retrospectivo, realizado entre dezembro de 2021 a agosto de 2022. A população estudada foi constituída de gestantes com Covid-19 que não foram imunizadas com vacina contra Covid-19 (n=91), em relação a grupo controle de gestantes não vacinadas sem indícios de infecção prévia ou atual por Covid-19 (n=57). Os testes utilizados para análise estatística foram: teste t para amostras independentes não pareadas, teste de Mann-Whitne, teste do Qui-Quadrado e teste exato de Fisher bi-caudado com extensão de Freeman-Halton. Como desfechos, pesquisou-se a internação em UTI e óbito de gestantes e recém-nascidos nos dois grupos. Resultados: encontrou-se uma maior tendência de internação em UTI e óbito nas gestantes com Covid-19 (RR= 9,4; 95% CI: 0,55 – 162,5) e (RR = 3,15; 95% CI: 0,15 – 64,50), respectivamente. Os recém-nascidos apresentaram maior risco de internação na UTI Neonatal (RR = 1,73; 95% CI: 0,73 – 4,14) e maior possibilidade de morte (RR = 2,45, 95% CI: 0,28 – 21,40).
