Surgical treatment of bilateral fracture of atrophic mandibula: Case report

Resumo: Mandibular atrophy occurs due to several factors such as periodontitis, periapical lesions, early tooth loss, use of poorly adapted prostheses that promote bone resorption over time. This condition provides fragility to the remaining bone structure and susceptibility to fractures. Atrophic mandible fractures represent a challenge to the Oral and Maxillofacial Surgeon due to several peculiarities in the management of treatment and oral rehabilitation. The treatment of choice for patients with atrophic mandible fracture is surgery, which aims to reduce and immobilize the fracture segments, in order to restore aesthetics and function. In this way, an individualized assessment must be carried out in search of the best biomechanics of the osteosynthesis material to be used, as adequate stability can be achieved through several fixation methods. The objective of this work is to report a clinical case of a male patient, victim of an accident with a large animal evolving with bilateral fracture of the atrophic mandible body. We opted for surgical treatment of the fracture with simplification of the fracture bone stumps using miniplates and subsequent installation of the load-bearing system plate, evolving in the postoperative period without complications.

Esta condición proporciona fragilidad a la estructura ósea restante y susceptibilidad a fracturas. Las fracturas atróficas de mandíbula representan un desafío para el Cirujano Oral y Maxilofacial debido a varias peculiaridades en el manejo del tratamiento y rehabilitación oral. El tratamiento de elección para los pacientes con fractura atrófica de mandíbula es la cirugía, que tiene como objetivo reducir e inmovilizar los segmentos de la fractura, con el fin de restaurar la estética y la función. Por tanto, se debe realizar una valoración individualizada en busca de la mejor biomecánica del material de osteosíntesis a utilizar, ya que se puede conseguir una estabilidad adecuada mediante varios métodos de fijación. El objetivo de este trabajo es reportar un caso clínico de un paciente masculino, víctima de un accidente con un animal de gran tamaño que evoluciona con fractura bilateral del cuerpo atrófico de la mandíbula. Optamos por el tratamiento quirúrgico de la fractura con simplificación de los muñones óseos de la fractura mediante miniplacas y posterior instalación de una placa de sistema portante, evolucionando en el postoperatorio sin complicaciones.A atrofia mandibular ocorre em virtude de diversos fatores como periodontite, lesões periapicais, perda dentária precoce, uso de próteses mal adaptadas que promovem a reabsorção óssea ao longo do tempo. Esta condição proporciona fragilidade à estrutura óssea remanescente e susceptibilidade á fraturas. As fraturas de mandíbula atrófica configuram-se um desafio ao Cirurgião Buco-Maxilo-Facial em virtude de diversas peculiaridades no manejo do tratamento e reabilitação oral. O tratamento de escolha para pacientes com fratura de mandíbula atrófica é o cirúrgico, que objetiva reduzir e imobilizar os segmentos da fratura, de modo a restaurar a estética e a função. Deste modo, uma avaliação individualizada deve ser realizada na busca da melhor biomecânica do material de osteossíntese a ser utilizado, pois uma adequada estabilidade pode ser conseguida através de diversos métodos de fixação. O objetivo deste trabalho é relatar um caso clínico de um paciente do gênero masculino, vítima de acidente com animal de grande porte evoluindo com fratura bilateral de corpo de mandíbula atrófica.
