Bibliographic survey of the main congenital heart disease associated with Down Syndrome in Brazil

Resumo: This research presents itself as a bibliographic review study with the main objective of understanding the main congenital heart diseases in patients with Down Syndrome and pointing out new updates on the subject. Down Syndrome (DS) is a genetic condition that can lead to severe physical and cognitive disabilities. Currently, most people with the disease do not have major difficulties in daily life and lead a relatively normal life as a result of the advancement and support of medicine. However, there are diseases that are more prevalent in DS when compared to the general population. With this, it is of great importance to point out what these diseases are and how they interfere in daily life so that these patients are provided with better living and health conditions. Congenital heart disease is the most prevalent comorbidity associated with Down Syndrome, with atrioventricular septal defect (AVSD) being the most common malformation. This alteration is caused as a result of the impediment of fusion of the embryonic endocardial cushions. Other malformations found in less prevalence are atrial septal defect (ASD), ventricular septal defect (IVC) and Tetralogy of Fallot. In-hospital mortality was significantly lower in populations undergoing cardiac intervention in heart diseases with lower proportions of cardiac arrest and less need for ECMO during hospitalization. The prevalence of heart disease in patients with Down Syndrome is notorious, however, further research is crucial to better understand its etiology.

El Síndrome de Down (SD) es una condición genética que puede conducir a severas discapacidades físicas y cognitivas. Actualmente, la mayoría de las personas que padecen la enfermedad no tienen mayores dificultades en la vida diaria y llevan una vida relativamente normal gracias al avance y apoyo de la medicina. Sin embargo, hay enfermedades que son más prevalentes en SD en comparación con la población general. Con esto, es de gran importancia señalar cuáles son estas enfermedades y cómo interfieren en la vida diaria para que estos pacientes cuenten con mejores condiciones de vida y salud. La cardiopatía congénita es la comorbilidad más prevalente asociada al síndrome de Down, siendo la comunicación auriculoventricular (AVSD, por sus siglas en inglés) la malformación más común. Esta alteración se produce como consecuencia del impedimento de fusión de las almohadillas endocárdicas embrionarias. Otras malformaciones encontradas con menor prevalencia son la comunicación interauricular (CIA), la comunicación interventricular (CIV) y la tetralogía de Fallot. La mortalidad hospitalaria fue significativamente menor en las poblaciones sometidas a intervención cardiaca en enfermedades cardiacas con menor proporción de parada cardiaca y menor necesidad de ECMO durante la hospitalización. La prevalencia de enfermedades del corazón en pacientes con Síndrome de Down es notoria, sin embargo, es fundamental realizar más investigaciones para comprender mejor su etiología.Esta pesquisa apresenta-se como um estudo do tipo revisão bibliográfica integrativa tendo como principal objetivo compreender as principais cardiopatias congênitas em pacientes portadores de Síndrome de Down e apontar novas atualizações mediante o assunto. A Síndrome de Down (SD) é uma condição genética que pode levar a graves incapacidades físicas e cognitivas. Atualmente, a maioria dos portadores da doença não possuem maiores dificuldades no cotidiano e levam uma vida relativamente normal em decorrência do avanço e suporte da medicina. Entretanto, há doenças que são mais prevalentes na SD quando comparado à população no geral. Com isso, é de grande importância pontuarmos quais são essas doenças e como elas interferem no dia a dia para que seja proporcionado a esses pacientes melhores condições de vida e saúde. A doença cardíaca congênita é a comorbidade associada mais prevalente à Síndrome de Down, sendo o defeito do septo atrioventricular (AVSD) a malformação mais encontrada. Essa alteração é causada em decorrência do impedimento de fusão dos coxins endocárdicos embrionários. Outras malformações encontradas em menos prevalência são comunicação interatrial (CIA), comunicação interventricular (VCI) e Tetralogia de Fallot. A mortalidade hospitalar foi significativamente menor nas populações submetidas a intervenção cardíaca nas cardiopatias com menores proporções de parada cardíaca e menor necessidade de ECMO durante a hospitalização.
